A coupling device includes a receiving part having a head receiving portion for pivotably receiving a head of a bone anchor and a rod receiving portion defining a recess for receiving the rod, the rod receiving portion having an engagement structure for engaging a locking member to lock the rod in the recess and a first engagement surface different from the engagement structure for engaging an instrument, wherein the first engagement surface is formed away from radially outwardly facing regions of the rod receiving portion, and a locking ring positionable around the head receiving portion and having a second engagement surface for engaging the instrument. The locking ring can assume an insertion position where the head of the bone anchor is insertable into the head receiving portion, and a pre-locking position where the head is prevented from removal from the head receiving portion.

What is claimed is: 1. A coupling device for coupling a rod to a bone anchor, the coupling device comprising: a receiving part having a first end and a second end below the first end, and comprising a head receiving portion at the second end defining a seat for pivotably receiving a head of the bone anchor, the head receiving portion being expandable to permit insertion of the head and compressible to lock the head in the seat, and a rod receiving portion defining a recess extending from the first end towards the second end and forming two legs for receiving the rod therebetween; and a locking ring positionable around the head receiving portion and movable from an insertion position where the head of the bone anchor is insertable into the head receiving portion to a pre-locking position where the head is prevented from removal from the head receiving portion; wherein the locking ring comprises an engagement surface configured to engage a first abutment on the rod receiving portion of the receiving part to restrict movement of the locking ring from the insertion position towards the first end of the receiving part and to engage a second abutment on the receiving part to restrict movement of the locking ring from the pre-locking position towards the insertion position. 2. The coupling device of claim 1 , wherein the locking ring is configured to further assume a locking position relative to the head receiving portion where the head is locked relative to the receiving part. 3. The coupling device of claim 1 , wherein a central passage extends through the receiving part from the rod receiving portion to the head receiving portion, and wherein the receiving part further comprises a first engagement portion that is accessible for engagement from the central passage by an instrument. 4. The coupling device of claim 3 , wherein the first engagement portion is formed on a surface of the rod receiving portion that defines the recess for the rod. 5. The coupling device of claim 3 , wherein the locking ring further comprises a second engagement portion that is accessible for engagement from the central passage by the instrument. 6. The coupling device of claim 1 , wherein the first abutment is circumferentially aligned with and positioned axially higher than the second abutment. 7. The coupling device of claim 1 , wherein the second abutment is also formed on the rod receiving portion of the receiving part. 8. The coupling device of claim 7 , wherein the first and second abutments are formed on one of the legs. 9. The coupling device of claim 1 , wherein the head of the bone anchor is insertable from the second end of the receiving part into the head receiving portion. 10. The coupling device of claim 1 , wherein an axial distance between the first and second abutments is non-adjustable. 11. A system for adjusting a coupling device of a bone anchoring device, the system comprising: the coupling device comprising a receiving part comprising a head receiving portion for pivotably receiving a head of a bone anchor, a rod receiving portion for receiving a rod, and a central passage extending through the receiving part from the rod receiving portion to the head receiving portion, and a locking ring positionable around the head receiving portion and movable from an insertion position where the head of the bone anchor is insertable into the head receiving portion to a pre-locking position where the head is prevented from removal from the head receiving portion; and an instrument having a longitudinal axis and comprising a first member configured to exert a pressure on the receiving part that is directed away from the head receiving portion, and a second member movable axially relative to the first member to engage the locking ring, wherein at least part of the instrument extends into the central passage of the receiving part when the instrument is engaged with the receiving part. 12. The system of claim 11 , wherein the first member of the instrument comprises a laterally extending projection or recess configured to engage a corresponding recess or projection on the receiving part. 13. The system of claim 11 , wherein at least part of the first member of the instrument extends into the central passage of the receiving part when the instrument is engaged with the receiving part. 14. The system of claim 11 , wherein at least part of the second member of the instrument extends into the central passage of the receiving part when the instrument is engaged with the locking ring. 15. The system of claim 14 , wherein the second member comprises a projection that extends laterally outwardly from the central passage to a position outside of the receiving part to engage the locking ring. 16. The system of claim 11 , wherein the locking ring further comprises a rod contacting surface configured to engage a rod received in the rod receiving portion, and wherein the second member of the instrument is configured to push against the rod contacting surface of the locking ring. 17. A system for placing a coupling device onto a head of a bone anchor, the system comprising: the coupling device comprising a receiving part comprising a head receiving portion for pivotably receiving the head of the bone anchor and a rod receiving portion for receiving a rod, and a clamping member movable relative to the head receiving portion to clamp the head, wherein the clamping member comprises a rod contacting surface configured to contact a rod received in the rod receiving portion; and an instrument having a longitudinal axis and comprising a first member engageable with the receiving part, and a second member movable axially relative to the first member and configured to directly engage the rod contacting surface of the clamping member to move the clamping member relative to the receiving part from an insertion position where the head of the bone anchor is insertable into the head receiving portion to a pre-locking position where the head is prevented from removal from the head receiving portion. 18. The system of claim 17 , wherein the first member of the instrument comprises a laterally extending projection or recess configured to engage a corresponding recess or projection on the receiving part. 19. The system of claim 17 , wherein the clamping member is formed as a locking ring positionable around the receiving part. 20. The system of claim 17 , wherein when the clamping member is at the pre-locking position, the clamping member is configured to block the head from being removed from the head receiving portion.
