Abstract

Official abstract text for this publication.

A system including a bone punch tool and a needle guide. The bone punch tool can include a support arm having a support arm proximal portion and a support arm distal portion, a pivot arm having a pivot arm proximal portion and a pivot arm distal portion, and an arcuate punch configured to punch through bone. The pivot arm distal portion can be pivotably coupled to the support arm distal portion, such that the pivot arm proximal portion is configured to be moved away from the support arm proximal portion to extend the arcuate punch into a punch position to punch an arcuate hole through bone. The needle guide can be configured to guide a needle through the arcuate hole.

First claim

Opening claim text (preview).

What is claimed is: 1. A method of closing a severed sternum following a sternotomy, the method comprising: providing or receiving a bracket having a plurality of bracket apertures each configured to receive a fastener; providing or receiving a guide unit having a plurality of fastener guides each configured to releasably hold the fastener; moving a first sternal bone portion towards a second sternal bone portion so as to close a thoracic cavity; positioning the bracket over the first and second sternal bone portions; positioning the guide unit over the bracket such that the plurality of fastener guides is aligned with the plurality of bracket apertures; and driving a plurality of fasteners through the guide unit into bracket apertures and into at least one of the first sternal bone portion and the second sternal bone portion. 2. The method of claim 1 , wherein each of the fastener guides is a tube having a proximal opening for loading the fastener into the tube, and a distal opening to allow for discharging the fastener from the tube. 3. The method of claim 2 , wherein the fastener guides have a same spacing and relative orientation as the bracket apertures such that the distal opening of each tube aligns with one of the bracket apertures when the guide unit is positioned over the bracket. 4. The method of claim 2 , wherein each tube is cylindrical in shape. 5. The method of claim 2 , wherein the fastener guides have a slit extending from the proximal opening down at least a portion of the tube. 6. The method of claim 1 , wherein the bracket apertures are circular and the fasteners are screws. 7. The method of claim 1 , further comprising: providing or receiving a cerclage; wrapping the cerclage around at least a portion of the each of the first and second sternal bone portions. 8. The method of claim 7 , wherein positioning the bracket comprises positioning the bracket over or adjacent the cerclage. 9. The method of claim 7 , further comprising: tensioning the cerclage to secure the first sternal bone portion to the second sternal bone portion. 10. The method of claim 7 , wherein the cerclage is a band formed of metallic or polymeric material. 11. The method of claim 7 , wherein two ends of the cerclage are twisted together to connect the two ends of the cerclage together. 12. The method of claim 1 , further comprising: coupling the guide unit to the bracket via a mounting feature. 13. The method of claim 1 , further comprising: separating the guide unit from the bracket while leaving the plurality of fasteners attached to the bracket. 14. The method of claim 1 , further comprising: preloading the guide unit with the plurality of fasteners prior to a surgical operation. 15. The method of claim 1 , wherein driving the plurality of fasteners comprises driving one of the plurality of fasteners into the first sternal bone portion and then driving a second of the plurality of fasteners into the second sternal bone portion. 16. The method of claim 1 , wherein positioning the bracket comprises placing the bracket such that it conforms to contours of at least one of the first and second sternal bone portions. 17. The method of claim 1 , wherein positioning the guide unit over the bracket comprises aligning a shape of the guide unit with at least a portion of a shape of the bracket. 18. A method of sternal closure; comprising: providing or receiving a bracket having a plurality of bracket apertures; providing or receiving a guide unit having a plurality of fastener guides; providing or receiving a plurality of fasteners; moving a first sternal bone portion towards a second sternal bone portion so as to reapproximate the first and second sternal bone portions; positioning the bracket over the first and second sternal bone portions; aligning the guide unit over the bracket such that each of the plurality of fastener guides of the guide unit aligns with a respective bracket aperture of the plurality of bracket apertures; engaging a driver to at least one fastener positioned in a fastener guide of the plurality of fastener guides; and driving the at least one fastener into a respective bracket aperture and into the first or second sternal bone portion. 19. The method of claim 18 , further comprising: preloading the guide unit with the plurality of fasteners prior to a surgical operation. 20. The method of claim 18 , further comprising: removing the guide unit from the bracket while the fastener remains extending through the bracket aperture into the first or second sternal bone portion.
