Concrete coupling assembly comprises a base holder for being attached to a concrete forming surface, the base holder including a central opening; a coupler having one end disposed within the central opening, the coupler having an axis for being positioned substantially transverse to the forming surface, the coupler having first and second threaded bores through the one end for attachment of a threaded rod; and a separate anchor body threadedly attached to another end of the coupler.

I claim: 1. A coupler, comprising: a) a body including a first end and an opposite second end, the body including a longitudinal axis; b) the body including a first axial threaded bore and a second axial threaded bore accessible from the first end, the first axial threaded bore having first diameter different from a second diameter of the second axial threaded bore; and c) the first end includes a peripheral beveled edge configured to indent a forming surface of a formboard to create a seal for the first and second axial threaded bores from concrete slurry. 2. The coupler as in claim 1 , wherein the second end includes an anchor head extending transversely from the longitudinal axis. 3. The coupler as in claim 2 , wherein the anchor head is hexagonal. 4. The coupler as in claim 3 , wherein the third axial threaded bore joins the second axial threaded bore. 5. The coupler as in claim 2 , wherein the anchor head includes a third axial threaded bore accessible from the second end. 6. The coupler as in claim 1 , wherein the body includes a third axial threaded bore accessible from the second end. 7. The coupler as in claim 6 , wherein the body includes a radial hole at a junction of the second axial threaded bore and the third axial threaded bore. 8. The coupler as in claim 1 , wherein the body is hexagonal in cross-section. 9. The coupler as in claim 1 , wherein the first end includes outside thread. 10. The coupler as in claim 1 , wherein the body includes an outside cylindrical surface. 11. The coupler as in claim 1 , wherein the body is a bolt. 12. The coupler as in claim 1 , wherein the anchor head is circular. 13. A coupler, comprising: a) a bolt including a body portion and a head portion, the body portion including outside thread, the bolt including a first end and an opposite second end, the bolt including a longitudinal axis; and b) the bolt including a first axial threaded bore and a second axial threaded bore accessible from the first end, the first axial threaded bore having a first diameter different from a second diameter of the second axial threaded bore. 14. The coupler as in claim 13 , wherein the head portion includes a hexagonal portion. 15. The coupler as in claim 13 , wherein the head portion includes a third axial threaded bore accessible from the second end. 16. The coupler as in claim 15 , wherein the third axial threaded bore joins the second axial threaded bore. 17. A coupler, comprising: a) a body including a first end and an opposite second end, the body including a longitudinal axis; b) the body including a first axial threaded bore and a second axial threaded bore accessible from the first end, the first axial threaded bore having first diameter different from a second diameter of the second axial threaded bore; c) the second end including an anchor head extending transversely from the longitudinal axis; and d) the anchor head including a third axial threaded bore accessible from the second end. 18. The coupler as in claim 17 , and further comprising a cap to cover the third axial threaded bore. 19. The coupler as in claim 17 , wherein the third axial threaded bore communicates with the first axial threaded bore and the second axial threaded bore.
