Concrete coupling assembly comprises a base holder for being attached to a concrete forming surface, the base holder including a central opening; a coupler having one end disposed within the central opening, the coupler having an axis for being positioned substantially transverse to the forming surface, the coupler having first and second threaded bores through the one end for attachment of a threaded rod; and a separate anchor body threadedly attached to another end of the coupler.

First claim

I claim: 1. A concrete anchor coupling assembly, comprising: a) a base holder for being attached to a concrete forming surface, the base holder including a threaded central opening; b) an anchor coupler having a body portion and a head portion, said body portion including a longitudinal axis and a first threaded bore along said axis, said head portion including a shoulder portion extending substantially transversely to said longitudinal axis of said body portion; and c) said body portion having an end portion being disposed within said threaded central opening, said end portion having outside threads for threaded engagement with said threaded central opening. 2. A concrete anchor coupling assembly as in claim 1 , wherein: a) said threaded central opening includes only a single thread; and b) said single thread is continuous or segmented. 3. A concrete anchor coupling assembly as in claim 1 , wherein said body portion includes a second threaded bore having a different diameter than said first threaded bore, said second threaded bore being along said axis. 4. A concrete anchor coupling assembly as in claim 3 , wherein said body portion includes a third threaded bore having a different diameter than said first or second threaded bore, said third threaded bore being along said axis. 5. A concrete anchor coupling assembly as in claim 3 , wherein said second threaded bore extends through said head portion. 6. A concrete anchor coupling assembly as in claim 5 , wherein said body portion includes a radial opening communicating with said second threaded bore. 7. A concrete anchor coupling assembly as in claim 5 , and further comprising a removable cap for closing said second threaded bore. 8. A concrete anchor coupling assembly as in claim 1 , wherein said head portion is circular. 9. A concrete anchor coupling assembly as in claim 1 , wherein said head portion is hexagonal. 10. A concrete anchor coupling assembly as in claim 1 , wherein said body portion is circular in cross-section. 11. A concrete anchor coupling assembly as in claim 1 , wherein said body portion is hexagonal in cross-section. 12. A concrete anchor coupling assembly, comprising: a) a base holder for being attached to a concrete forming surface, the base holder including central opening; b) an anchor coupler having a body portion and a head portion, said body portion including a longitudinal axis and a first threaded bore along said axis, said head portion including a shoulder portion extending substantially transversely to said longitudinal axis of said body portion; c) said body portion having an end portion being disposed within said central opening; d) said base holder including a stop member at a bottom of said central opening and extending into said central opening to prevent a bottom edge of said end portion from extending beyond said bottom of said central opening; e) a removable cap having a stem and a flange, said stem is disposed inside said first threaded bore; and f) said stop member is sandwiched between said bottom edge and said flange. 13. A concrete anchor coupling assembly as in claim 12 , wherein said body portion includes a second threaded bore having a different diameter than said first threaded bore, said second threaded bore being along said axis. 14. A concrete anchor coupling assembly as in claim 13 , wherein said body portion includes a third threaded bore having a different diameter than said first or second threaded bore, said third threaded bore being along said axis. 15. A concrete anchor coupling assembly as in claim 14 , wherein: a) said first threaded bore extends through said end portion of said body portion; b) said second threaded bore is disposed next to said first threaded bore, said second threaded bore having a diameter smaller than a diameter of the first threaded bore; and c) said third threaded bore is disposed next to said second threaded bore, said third threaded bore having a diameter smaller than a diameter of said second threaded bore. 16. A concrete anchor coupling assembly as in claim 12 , wherein said body portion includes a second threaded bore extending through said head portion. 17. A concrete anchor coupling assembly as in claim 12 , wherein said body portion is hexagonal in cross-section. 18. A concrete anchor coupling assembly as in claim 12 , wherein said head portion is threaded to said body portion. 19. A concrete anchor coupling assembly as in claim 12 , wherein said first threaded bore is a threaded opening of a split nut disposed within said body portion. 20. A concrete anchor coupling assembly as in claim 12 , wherein said stem is threaded and threadedly attached to said first threaded bore.
