Described herein are methods, systems and compositions for the diagnosis, prognosis and treatment of dementia and Alzheimers disease. Also described are methods, systems and compositions to distinguish between Alzheimers disease and Parkinsons disease. In various embodiments levels of PACAP and/or SIRT3 are analyzed for the diagnosis, prognosis and treatment of dementia and Alzheimers disease.

What is claimed is: 1. A method of treating late onset Alzheimer' s disease in an individual, comprising: providing a composition comprising PACAP, analogs thereof, salts thereof, or combinations thereof, and a pharmaceutically acceptable carrier; and administering a therapeutically effective dosage of the composition to the individual who has been determined to have late onset Alzheimer's disease based on a level of PACAP protein present in the individual's cerebrospinal fluid. 2. The method of claim 1 , the individual has been determined to have a PACAP protein level in the individual's cerebrospinal fluid below a reference value. 3. A method for treating late onset Alzheimer's disease in a subject, comprising: obtaining the results of an analysis of a PACAP protein level in a subject's cerebrospinal fluid; and administering a composition comprising PACAP to the subject when the PACAP protein level in the subject's cerebrospinal fluid is below a reference value. 4. The method of claim 3 , wherein the method comprises obtaining the results of an analysis for both a PACAP level and a SIRT3 level in the subject's cerebrospinal fluid.
