Provided are omecamtiv mecarbil dihydrochloride salt forms, compositions and pharmaceutical formulations thereof, and methods for their preparation and use. Omecamtiv mecarbil (AMG 423, CK-1827452), having the structure:

What is claimed: 1. A dihydrochloride monohydrate salt of omecamtiv mecarbil. 2. A dihydrochloride monohydrate salt of omecamtiv mecarbil, wherein the salt is crystalline and is characterized by an X-ray powder diffraction pattern comprising peaks at about 6.6, 14.9, 20.1, 21.4, and 26.8±0.2° 2θ using Cu Kα radiation. 3. The salt of claim 2 , wherein the X-ray powder diffraction patter further comprises peaks at about 8.4, 24.2, 26.0, and 33.3±0.2° 2θ using Cu Kα radiation. 4. The salt of claim 2 , wherein the X-ray powder diffraction patter further comprises peaks at about 6.2, 9.7, 13.2, 14.3, 15.4, 16.3, 16.9, 18.9, 19.5, 20.7, 21.8, 22.8, 23.6, 25.1, 27.3, 27.7, 28.4, 29.4, 30.2, 31.2, 31.5, 31.9, 33.9, 34.5, 34.9, 36.1, 36.8, 37.7, 38.5, and 39.7±0.2° θ using Cu Kα radiation. 5. The salt of claim 2 , having an X-ray powder diffraction pattern substantially as shown in FIG. 2 . 6. The salt of claim 2 , having an endothermic transition at about 230° C. to about 240° C., as measured by differential scanning calorimetry. 7. The salt of claim 6 , wherein the endothermic transition is at about 235° C.
