What is claimed is: 1. A surgical positioning system, comprising: a first component including an elongate shaft coupled to a head, the head configured to be disposed in a joint between a first bone and a second bone; a second component including diverging first and second portions, the first portion defining a hole that is sized and configured to receive the shaft of the first component, the second portion defining a first channel on a first side, the first channel being defined by a bottom wall and a pair of spaced apart side walls that extend from the bottom wall and defining an axis, wherein the side walls extend from the bottom wall at a non-orthogonal angle; and a third component configured to be coupled to the second component, the third component including a base and a pointer extension, the base including a protrusion that is sized and configured to be received slidably within the first channel such that the third component can translate with respect to the first component and the second component along the axis, the pointer extension extending in a direction substantially perpendicular to the axis. 2. The surgical positioning system of claim 1 wherein the second component defines a second channel on a second side, and wherein the protrusion of the third component is configured to be received slidably within the second channel. 3. The surgical positioning system of claim 2 , wherein the second channel is defined by a bottom wall and a pair of spaced apart side walls that extend from the bottom wall. 4. The surgical positioning system of claim 1 , wherein the pointer extension defines a hole along its length that is sized and configured to receive a pin therein. 5. The surgical positioning system of claim 1 , wherein the head includes a first prong and a second prong. 6. The surgical positioning system of claim 5 , wherein the first and second prongs are sized and configured to be received within a medial gutter of an ankle joint. 7. The surgical positioning system of claim 1 , wherein the hole defined by the first portion is configured to receive the shaft of the first component rotatably therein. 8. A surgical positioning system, comprising: a medial gutter fork configured to be disposed in a joint between a tibia and a talus; a rotation guide slide including diverging first and second portions, the first portion defining a hole that is sized and configured to receive the shaft of the medial gutter fork, the rotation guide slide defining a first channel on a first side and comprising a flat bottom surface and angled side walls tapered inwardly such that a top of the first channel is narrower than a bottom, the first channel defining an axis; and a rotation guide pointer configured to be coupled to the rotation guide slide, the rotation guide pointer including a base and a pointer extension, the base including a protrusion that is sized and configured to be received slidably within the first channel such that the rotation guide pointer can translate with respect to the medial gutter fork and the rotation guide slide along the axis, the pointer extension extending in a direction substantially perpendicular to the axis. 9. The surgical positioning system of claim 8 , wherein the rotation guide pointer comprises a finger tab extending from the base along a lateral axis perpendicular to a longitudinal direction of the protrusion. 10. The surgical positioning system of claim 8 , wherein the diverging first and second portions of the rotation guide slide are perpendicular. 11. The surgical positioning system of claim 8 , wherein the pointer extension comprises a pin hole formed therein, wherein the pin hole comprises a guide for placement of a first guide pin.
