Methods and apparatus, including computer program products, are provided for remote monitoring. In some example implementations, there is provided a method. The method may include receiving, at a remote monitor, a notification message representative of an event detected, by a server, from analyte sensor data obtained from a receiver monitoring an analyte state of a host; presenting, at the remote monitor, the notification message to activate the remote monitor, wherein the remote monitor is configured by the server to receive the notification message to augment the receiver monitoring of the analyte state of the host; accessing, by the remote monitor, the server, in response to the presenting of the notification message; and receiving, in response to the accessing, information including at least the analyte sensor data. Related systems, methods, and articles of manufacture are also disclosed.

What is claimed is: 1. A method for remotely monitoring glucose concentration for a plurality of hosts by any number of remote wireless mobile computing devices in communication with a remote server system, the remote server system storing glucose concentration data for each of the plurality of hosts, the glucose concentration data of each host being measured by a different glucose monitoring system, the method comprising: authorizing, at the remote server system, a request from a first remote monitoring application installed on a first remote wireless computing device to receive glucose information of a first host; authorizing, at the remote server system, a request from a second remote monitoring application installed on a second remote wireless computing device to receive glucose information of a second host; establishing, at the remote server system, a plurality of rule sets, a first rule set of the plurality of rule sets being defined by the first host and that can be modified by a first user using the first remote monitoring application, a second rule set of the plurality of rule sets being defined by the second host and that can be modified by a second user using the second remote monitoring application, wherein the first rule set comprises a first threshold value associated with a first host's glucose concentration and wherein the second rule set comprises a second threshold value associated with a second host's glucose concentration, and wherein the first threshold value is different from the second threshold value; determining, by the remote server system, that the at least some glucose concentration data indicative of the first host's glucose concentration received from the first host's glucose monitoring system exceeds or is below the first threshold value; generating, by the remote server system, a first alert informative of an event based on the determination that the at least some of the glucose concentration data indicative of the first host's glucose concentration exceeds or is below the first threshold value; transmitting, by the remote server system, the first alert to the first remote wireless computing device, wherein the first remote wireless computing device is configured to display the notification alert regardless of whether the first remote monitoring application is active or inactive on the first remote wireless computing device and activate the first remote monitoring application if the first remote monitoring application is inactive; receiving and confirming, by the remote server system, a first acknowledgement from the first remote monitoring application indicating that the first user has received the first alert, the remote server system not providing additional glucose concentration data indicative of the first user's glucose concentration to the first remote monitoring application until the first acknowledgement is confirmed; determining, by the remote server system, that the at least some glucose concentration data indicative of the second host's glucose concentration received from the second host's glucose monitoring system exceeds or is below the second threshold value; generating, by the remote server system, a second alert informative of an event based on the determination that the at least some of the glucose concentration indicative of the second host's glucose concentration data exceeds or is below the second threshold value; transmitting, by the remote server system, the second alert to the second remote wireless computing device, wherein the second remote wireless computing device is configured to display the alert regardless of whether the second remote monitoring application is active or inactive on the second remote wireless computing device and activate the second remote monitoring application if the second remote monitoring application is inactive; and receiving and confirming, by the remote server system, a second acknowledgement from the second remote monitoring application indicating that the second user has received the second alert, the remote server system not providing additional glucose concentration data indicative of the second user's glucose concentration to the second remote monitoring application until the second acknowledgement is confirmed. 2. The method of claim 1 , wherein the first and second remote wireless computing devices are smartphones. 3. The method of claim 1 , wherein the first alert is a text message. 4. The method of claim 1 , wherein the first host's glucose monitoring system comprises a continuous glucose sensor, a sensor electronics module attached to the glucose sensor, and a receiver in wireless communication with the sensor electronics module, wherein the sensor electronics module intermittently transmits the at least some of the glucose concentration data indicative of the first host's glucose concentration to the receiver; and the receiver intermittently transmits the at least some of the glucose concentration data indicative of the first host's glucose concentration to the remote server. 5. The method of claim 4 , wherein the receiver is a smart mobile communication device with an application downloaded thereon to receive sensor data from the transmitter and transmit the sensor data to the remote server. 6. The method of claim 4 , further comprising: applying, by the receiver, a plurality of predefined thresholds to the sensor data; and triggering a user-perceptible alarm when the sensor data satisfies one or more of the plurality of predefined thresholds. 7. The method of claim 1 , further comprising configuring the first rule set of the plurality of rule sets based on configuration parameters received from the first remote monitoring application, the configuration parameters including the first threshold value. 8. The method of claim 1 , the first rule set additionally comprises a loss of data rule, the method further comprising: determining, by the remote server system, if the loss of data rule is satisfied when the remote server system does not receive the glucose concentration data indicative of the first host's glucose concentration from the first host's glucose monitoring system for a predetermined amount of time; transmitting, by the remote server system, a third alert indicating loss of data from the first host's glucose monitoring system to the first remote wireless computing device, the first host's glucose monitoring system, or both. 9. The method of claim 8 , wherein the predetermined amount of time for the first rule set is user-configurable by the first remote monitoring application. 10. The method of claim 1 , wherein the first rule set is configurable by the first remote wireless computing device but not the second remote wireless computing device, and the second rule set is configurable by the second remote wireless computing device but not the first remote wireless computing device. 11. The method of claim 1 , wherein the first concentration value and second concentration values are hypoglycemia thresholds, wherein the hypoglycemia threshold for the first concentration value is different than the hypoglycemia threshold for the second concentration value. 12. The method of claim 1 , wherein the first concentration value and second concentration values are hyperglycemia thresholds, wherein the hyperglycemia threshold for the first concentration value is different than the hyperglycemia threshold for the second concentration value. 13. The method of claim 1 , wherein each of the first threshold value and the second threshold value thresholds comprise a threshold selected from the group consisting of a low glucose threshold, a high glucose threshold and a gluco
