Abstract

Official abstract text for this publication.

A stent graft ( 40 ) for treating Type-A dissections in the ascending aorta ( 22 ) is provided with a plurality of diameter-reducing suture loops ( 56 - 60 ) operable to constrain the stent graft during deployment thereof in a patient's aorta. The diameter-reducing loops ( 56 - 60 ) allow the stent graft ( 40 ) to be partially deployed, in such a manner that its location can be precisely adjusted in the patient's lumen. In this manner, the stent graft can be placed just by the coronary arteries ( 26, 28 ) with confidence that these will not be blocked. The stent graft ( 40 ) is also provided with proximal and distal bare stents ( 44,52 ) for anchoring purposes.

We claim: 1. A stent graft system configured for treatment of defects in the ascending aorta with a stent graft, the stent graft comprising a graft tube comprising a tubular portion of graft material, the graft tube provided with proximal and distal ends, the stent graft further comprising a plurality of stent rings attached to the graft tube, a first mechanically releasable diameter-restraining device located at or proximate the proximal end of the graft tube and restraining the proximal end of the graft tube, a second mechanically releasable diameter-restraining device restraining a proximal end of one of the plurality of stent rings located at an intermediate position between two other of the plurality of stent rings located on either side of the at least one of the plurality of stent rings located at the intermediate position, and a third mechanically releasable diameter-restraining device restraining a distal end of one of the plurality of stent rings located at the intermediate position along the graft tube, at least one release mechanism operable to release the second and third mechanically releasable diameter-restraining devices, and a bare stent extending from the proximal end of the graft tube and designed to flare outwardly relative to the graft tube so as to engage in use a cavity of an aortic sinus of a patient, where the stent graft has a first expanded configuration and a second expanded configuration where the stent graft is fully expanded, where in the first expanded configuration the graft tube at the intermediate position has a smaller diameter than adjacent portions of the graft tube. 2. A stent graft system according to claim 1 , further comprising a bare stent extending from the distal end of the graft tube. 3. A stent graft system according to claim 2 , wherein the distal bare stent flares outwardly relative to the graft tube. 4. A stent graft system according to claim 1 , wherein the bare stent is formed of an undulating stent structure comprising a series of fingers arranged circumferentially around the graft tube and having curved ends or apices. 5. A stent graft system according to claim 1 , wherein the graft tube is about 65 mm in length. 6. A stent graft system according to claim 1 , wherein the bare stent extends from the graft tube by about 10 mm. 7. A stent graft system according to claim 1 , wherein the second diameter-restraining device is located substantially half-way along the graft tube. 8. A stent graft system according to claim 1 , wherein the first, second, and third mechanically releasable diameter-restraining devices are diameter-reducing loops disposed circumferentially around the graft tube. 9. A stent graft system according to claim 1 , wherein the stent graft comprises a stent section extending from the distal end of the graft tube. 10. A stent graft system according to claim 9 , wherein the stent section is one of integral with the graft tube or formed as a separate component deployable after deployment of the graft tube. 11. A stent graft system of claim 1 , further including an introducer assembly, the introducer assembly including a carrier for carrying the stent graft, an outer sheath movable from a position covering the carrier to a withdrawn position exposing the carrier; wherein the at least one release mechanism mechanically releasably maintains the first, the second, and the third mechanically releasable diameter restraining devices of the stent graft in a radially compressed configuration after withdrawal of the sheath. 12. The stent graft system of claim 11 , wherein the at least one release mechanism includes one or more trigger wires. 13. The stent graft system of claim 1 , further comprising a fourth mechanically releasable diameter-restraining device restraining a proximal end of second of the plurality of stent rings located at the intermediate position and a fifth mechanically releasable diameter-restraining device restraining a distal end of the second of the plurality of stent rings located at the intermediate position. 14. A stent graft system comprising a stent graft including a tube of graft material, the tube of graft material having proximal and distal ends and an intermediate position between the proximal and distal ends, a plurality of stent rings attached to the tube of graft material, with one of the plurality of stent rings located at the intermediate position and between two other of the plurality of stent rings, and at least first, second, and third mechanically releasable diameter-restraining devices, the first mechanically releasable diameter-restraining device located proximate the proximal end of the tube of graft material and restraining the proximal end of the tube of graft material, the second mechanically releasable diameter-restraining device being located at the intermediate position and restraining the stent ring located at the intermediate position, at least one non-expandable release mechanism engaging at least one of the first and second mechanically releasable diameter restraining devices, where the stent graft has a first expanded configuration and a second expanded configuration, where in the first expanded configuration the stent graft, at the intermediate position, is contracted and has a smaller diameter than adjacent portions of the stent graft, and wherein the second mechanically releasable diameter-restraining device restrains the proximal end of the stent ring located at the intermediate position of the tube of graft material and the third diameter-restraining device restrains the distal end of the stent ring located at the intermediate position of the tube of graft material. 15. A stent graft system according to claim 14 , wherein the second and third mechanically releasable diameter-restraining devices are configured as diameter-reducing loops disposed circumferentially around the tube of graft material. 16. The stent graft system of claim 14 , wherein the at least one non-expandable release mechanism is a trigger wire.
