An anchor assembly for use in spinal fixation to interconnect a longitudinal spinal rod, which is integrally formed with a body of the anchor assembly. The anchor assembly includes a bone anchor, a collet, a body portion, a rod portion, and a locking cap. The anchor assembly is preferably configured so that in use, the bone anchor may be secured to the patient's vertebra prior to being received within the body portion. The anchor assembly enables a surgeon to implant the bone anchor without the body portion to maximize visibility and access around the anchoring site. Once the bone anchor has been secured to the patient's vertebra, the rod portion may be inserted into the rod-receiving channel of a second bone fixation element having a rod-receiving channel implanted at a second site and the body portion can be snapped-onto the bone anchor.

The invention claimed is: 1. A bone anchor assembly configured to stabilize bones or bone fragments, the bone anchor assembly comprising: a body having an upper end, a lower end spaced from the upper end along a longitudinal axis, a rod that projects from the body along a transverse direction that is substantially perpendicular to the longitudinal axis, the body defining a bore that extends from the upper end to the lower end, a first spherical surface adjacent to the lower end of the body, and a second spherical surface disposed between the first spherical surface and the upper end of the body, wherein the first spherical surface has a first radius of curvature defined with respect to the longitudinal axis; a collet inside the bore and moveable from a first position in the bore to a locked position in the bore adjacent the lower end of the body, the collet having a first end, a second end, and a plurality of flexible arms defined by slots that extend from the second end toward the first end, the flexible arms defining an outer spherical surface, the outer spherical surface having a distal portion adjacent to the second end of the collet and a proximal portion spaced from the distal portion and the first end of the collet, wherein the outer spherical surface has a collet radius of curvature that is different than the first radius of curvature of the first spherical surface such that the distal portion is in contact with the first spherical surface yet the proximal portion is spaced from the first spherical surface when the collet is in the locked position; and wherein the second spherical surface has a second radius of curvature defined with respect to the longitudinal axis that is greater than the first radius of curvature of the first spherical surface. 2. The bone anchor assembly of claim 1 , wherein the rod is a spinal rod. 3. The bone anchor assembly of claim 2 , wherein the spinal rod is elongate along a rod central axis that is aligned with the transverse direction, the rod central axis extending through the body and intersecting the longitudinal axis, wherein the collet is supported by the body such that the rod central axis extends through the collet when the collet is in at least one of the first position and the locked position. 4. The bone anchor assembly of claim 3 , wherein the spinal rod central axis extends through the collet when the collet is in either the first position or the locked position. 5. The bone anchor assembly of claim 3 , wherein the body defines an outer surface and the outer surface at least partially defines the spinal rod. 6. The bone anchor assembly of claim 1 , wherein the bore has a first diameter proximate the upper end and a second diameter proximate the lower end that is smaller than the first diameter, and wherein the second spherical surface defines an enlarged portion having a third diameter that is greater than the first diameter. 7. The bone anchor assembly of claim 6 , wherein the first position is when the collet is substantially aligned with the enlarged portion along the longitudinal axis such that the flexible arms radially expand toward the second spherical surface, and the locked position is when a portion of the first spherical surface radially compresses the flexible arms toward the longitudinal axis. 8. The bone anchor assembly of claim 1 , wherein the flexible arms have a terminal end at the second end of the collet, and wherein at least the terminal ends of the flexible arms are in contact with at least a portion of the first spherical surface adjacent to the lower end when the collet is in the locked position and oriented along the longitudinal axis. 9. The bone anchor assembly of claim 1 , further comprising a bone anchor including a head, wherein the head includes a tool interface configured to engage a surgical instrument operatively associated with the bone anchor. 10. The bone anchor assembly of claim 9 , wherein when the collet is in the first position, the head of the bone anchor is receivable within the flexible arms, and when the collet is in locked position, the head of the bone anchor is locked by the flexible arms relative to the body. 11. The bone anchor assembly of claim 9 , wherein the collet defines a cavity adjacent to the second end, wherein the head of the bone anchor is polyaxially moveable within the cavity when the collet is in the first position. 12. The bone anchor assembly of claim 1 , further comprising a locking cap configured for moveable engagement with the upper end of the body, the cap moveable from a first unlocked position to a second locked position, wherein movement of the locking cap from the first unlocked position to the second locked position causes the collet to move from the first position to the locked position. 13. The bone anchor assembly of claim 12 , wherein the locking cap has an exterior thread, the exterior thread defining a first thread pitch, and wherein the body defines an inner surface at least partially defining the bore, wherein the inner surface defines an inner thread having a second thread pitch. 14. The bone anchor assembly of claim 13 , wherein the first thread pitch is different than the second thread pitch. 15. The bone anchor assembly of claim 14 , wherein the second thread pitch is at least about 1.15 mm. 16. A pedicle screw bone anchor assembly for receiving a head of a first pedicle screw bone anchor for use in a system with a second pedicle screw bone anchor and second pedicle screw anchor assembly in conjunction with a spinal rod, the pedicle screw bone anchor assembly comprising: a body having an upper end, a lower end spaced from the upper end along a longitudinal axis, a spinal rod that extends along a transverse direction that is substantially perpendicular to the longitudinal axis, the spinal rod elongate along a rod central axis that is aligned with the transverse direction, the spinal rod having a length sufficient to extend across an intervertebral disc space spanning a first vertebra and an adjacent second vertebra and the spinal rod central axis intersecting the longitudinal axis, the body defining a bore that extends from the upper end to the lower end, a lower spherical surface adjacent to the lower end of the body and an enlarged portion spaced from the lower spherical surface toward the upper end; and a collet in the bore and moveable between a loading position and a locked position, the collet having a first end, a second end, and at least two slots extending from the second end toward the first end, the at least two slots defining at least two flexible arms, wherein each flexible arm has a root end and a terminal end, the at least two flexible arms radially expand when the collet is in the loading position and radially contract when the collet is in the locked position, and at least the terminal ends of the flexible arms are in contact with at least a portion of the lower spherical surface adjacent to the lower end when the collet is in the locked position and oriented along the longitudinal axis, wherein the rod central axis extends through the collet when the collet is in at least one of the locked position and the loading position; wherein the bore has a first diameter at the upper end, a second diameter at the lower end, and a third diameter at the enlarged portion, wherein the third diameter is greater than the first diameter and the first diameter is greater than the second diameter; and wherein the spinal rod is formed integrally with the body. 17.
