Official abstract text for this publication.
An apparatus for the efficient provision of directed illumination to a subdermal surgical field that is attachable to or completely forms a surgical retractor blade. The surgical illumination system may include a retractor having a proximal handle portion and a distal portion adapted for insertion into a surgical field and retraction of body tissue, and a light guide disposed on the retractor.

What is claimed is: 1. A medical system for illuminating a surgical field, said system comprising: a light guide formed of a light conducting material, the light guide having proximal and distal ends; a flexible input formed of a light conducting material, the flexible input coupled to the proximal end of the light guide; a plurality of micro structures disposed along a longitudinal axis of the light guide, the plurality of micro structures configured to shape light passing therethrough and direct the light onto the surgical field while reducing shadowing; and at least some of the plurality of microstructures form at least two stair steps each having a riser surface and a step surface, the riser surface and the step surface cooperatively defining an angle therebetween, wherein the angle changes from one stair step in the at least two stair steps to another stair step in the at least two of stair steps so that light is emitted from the one stair step at a first angle and light emitted from the another stair step is emitted at an increased angle relative to the light emitted at the first angle. 2. The system of claim 1 , further comprising a retractor adapted to retract tissue in the surgical field. 3. The system of claim 2 , wherein the light guide is engaged with the retractor. 4. The system 2 , wherein the retractor comprises grooves disposed therealong, the grooves configured to slidably receive the light guide. 5. The system of claim 2 , wherein the light guide is integrally formed with the retractor. 6. The system of claim 1 , wherein the flexible input comprises a fiber optic cable. 7. The system of claim 1 , wherein the flexible input is flared where the flexible input is attached to the light guide so as to distribute light across a width of the light guide. 8. The system of claim 1 , wherein a proximal end of the flexible input comprises a length of flexible material with an optical connector coupled thereto, the optical connector adapted to be coupled to a light source. 9. The system of claim 1 , wherein the light guide has a width and a thickness, and wherein the width increases from the proximal end of the light guide toward the distal end of the light guide. 10. The system of claim 9 , wherein the thickness decreases from the proximal end to the distal end of the light guide. 11. The system of claim 1 , wherein the light guide comprises a proximal portion and a distal portion disposed transversely to the proximal portion. 12. The system of claim 11 , wherein the flexible input is disposed on the proximal portion of the light guide. 13. The system of claim 12 , wherein the plurality of micro structures is disposed on the distal portion of the light guide. 14. The system of claim 1 , wherein spacing of the plurality of micro structures is irregular or aperiodic. 15. The system of claim 1 , wherein a length of the riser surface of a first micro structure of the plurality of micro structures is different than a length of the riser surface of a second micro structure of the plurality of micro structures. 16. The system of claim 1 , wherein a length of the step surface of a first micro structure of the plurality of micro structures is different than a length of the step surface of a second micro structure of the plurality of micro structures. 17. The system of claim 1 , wherein one or more of the step surfaces or the flat riser surfaces of the plurality of micro structures are coated to promote total internal reflection. 18. The system of claim 1 , wherein light emitted from the one stair step illuminates a first area of the surgical field and light emitted from the another stair step illuminates a second area of the surgical field different than the first area. 19. The system of claim 1 , wherein the light emitted from the another stair step forms a wider beam relative to the light emitted from the one stair step.
