An alignment system for aligning an acetabular cup insertion instrument utilizes pre-operative preferably standing x-rays and intra-operative x-rays to allow the surgeon to compensate for the position of a patient on an operating room table. The system uses a programmable computer connected to a digital x-ray system and an navigation tracking system to provide input for calculating the inclination and anteversion angles of an acetabular cup impactor based on a pre-operative plan developed from the standing x-rays. The system calculates changes in lengths and angles between anatomic landmarks on the pelvis to alter the intra-operative orientation of the acetabular reaming and impacting instruments to produce the desired inclination and anteversion angles when the pelvis is reoriented as the patient is placed on an operating table.

The invention claimed is: 1. A method for aligning an acetabular cup insertion instrument comprising: taking a pre-operative anterior/posterior view digital x-ray image and a lateral view pre-operative digital x-ray image of the pelvis of a patient; determining a desired acetabular cup abduction and anteversion, angle based on a pre-operative x-ray; identifying at least three points on the digital anterior/posterior digital x-ray image and at least two points on the lateral digital x-ray image; calculating the lengths and angles between each of the points on both the anterior/posterior pre-operative digital x-ray image and the lateral pre-operative digital x-ray image; positioning a patient on an operating table in an operating room, the operating table having a reference element thereon; taking at least one intra-operative pelvic digital x-ray image of the patient positioned on the operating table; identifying at least three points on the intra-operative x-ray image, the points corresponding to the points on the pre-operative x-ray image; calculating the lengths and angles between each of the at least three points on the intra-operative digital x-ray image; calculating an intra-operative angular deviation of the acetabular cup insertion instrument from the desired acetabular cup abduction and anteversion angle by comparing the dimensional differences between the points on the pre-operative x-ray images and the at least one intra-operative x-ray image; and intra-operatively aligning the acetabular cup insertion instrument to the calculated angular position, based on the intra-operative angular deviation from the desired abduction and anteversion angles, using angle measuring tools. 2. The method as set forth in claim 1 wherein the pre-operative x-rays are standing x-rays. 3. The method as set forth in claim 1 wherein the at least three points on the anterior/posterior pre-operative and intra-operative images are the right and left promontory points and the pubic symphysis. 4. The method as set forth in claim 3 further comprising taking an intra-operative later x-ray image and identifying the at least two points on the intra-operative lateral image are the left or right promontory and the public symphysis. 5. The method as set forth in claim 1 wherein additional points are selected on the pre and post-operative x-rays from the group consisting of the acetabular teardrops, the obturator frame and the base of the left and right ischial rings, the points being identified on the pre-operative and intra-operative images. 6. The method as set forth in claim 5 wherein the angle between a line connecting the ischial ring points and the reference element indicates any obliquely change between the pelvic anterior/posterior x-ray image and the at least one intra-operative x-rays. 7. The method as set forth in claim 1 wherein the reference element is a radiolucent bar extending parallel to the operating table surface, the radiolucent bar having two radiopaque markers thereon. 8. The method as set forth in claim 7 wherein the radiolucent bar has three radiopaque markers, each marker located at an apex of a triangle. 9. The method as set forth in claim 1 wherein the pre-operative determination of the desired cup abduction and anteversion angles based on the x-ray are about 30 to 50 degrees and 10 to 30 degrees respectively. 10. The method as set forth in claim 1 wherein the calculations are performed by a computer using digital image analysis software receiving input from a digital x-ray machine and an operating room navigation tracking system. 11. The method as set forth in claim 1 wherein a pelvic tilt is calculated by comparing the distance between the promontory points and the pubic symphysis on both the pre-operative x-rays and the at least one intra-operative x-ray. 12. The method as set forth in claim 1 wherein the pre-operative x-ray plane is recreated intra-operatively and is used as the plane to apply cup inclination in the plane and to apply cup version. 13. A method for aligning an acetabular cup insertion instrument comprising: taking a pre-operative anterior/posterior view digital x-ray image and a lateral view pre-operative digital x-ray image of the pelvis of a patient; determining a desired acetabular cup abduction and anteversion angle; identifying at least three anatomic points on the digital anterior/posterior digital x-ray image and at least two points on the pre-operative lateral digital x-ray image; calculating the lengths and angles between each of the points on both the anterior/posterior pre-operative digital x-ray image and the pre-operative lateral digital x-ray image; positioning a patient on an operating table in an operating room, the operating table having a reference element thereon, the operating room having a navigation system therein, the operating table having a navigation tracker mounted in a known position with respect to the operating table; taking at least one pelvic digital x-ray image of the patient positioned on the operating table, the x-ray including the reference element; identifying at least three points on the intra-operative x-ray image, the points corresponding to the points on the pre-operative x-ray image; calculating the lengths and angles between each of the at least three points on the intra-operative digital x-ray image; calculating the intra-operative pelvic tilt, obliquity and rotation based on the changes of the lengths and angles between the points on the pre and intra-operative digital x-rays; calculating an intra-operative angular position of the acetabular cup insertion instrument to produce the desired abduction and anteversion angle by comparing the pre-operative tilt, obliquity and rotation to the intra-operative tilt, obliquity and rotation; and aligning the insertion instrument to the calculated angular position using the navigation camera and a navigation tracker mounted on the insertion instrument. 14. The method as set forth in claim 13 wherein the at least three points on the anterior/posterior pre-operative and intra-operative images are the right and left promontory points and the pubic symphysis. 15. The method as set forth in claim 14 wherein the at least two points on the intra-operative lateral image are the left or right promontory and the public symphysis. 16. The method as set forth in claim 13 wherein additional points are selected from the group consisting of acetabular teardrops, the obturator frame and the base of the left and right ischial rings, the points being identified on the pre-operative and intra-operative images. 17. The method as set forth in claim 16 wherein the angle between a line connecting the ischial ring points and the reference element indicates any obliquely change between the pre-operative pelvic anterior/posterior x-ray image and the at least one intra-operative x-rays. 18. The method as set forth in claim 13 wherein the reference element is a radiolucent bar extending parallel to the operating table surface, the radiolucent bar having two radiopaque markers thereon. 19. The method as set forth in claim 18 wherein the radiolucent bar has three radiopaque markers, each marker located at an apex of a triangle. 20. The method as set forth in claim 13 wherein the pre-operative determination of the desired cup abduction and anteversion angles based on the pre-operative x-ray are about 40 to 45 degrees and 15 to 20 degrees respectively. 21.
