A plate-shaped ultrasonic motor for use in an auto-focus driving part or an OIS driving part of a lens drive apparatus, includes: a plate-shaped resonant part disposed in parallel to an optical axis; a pair of plate-shaped piezoelectric elements laminated to the plate-shaped resonant part from both sides thereof in a plate-thickness direction; and a single electrode having a pair of plate-shaped terminal parts sandwiching the pair of piezoelectric element from both sides in the plate-thickness direction to distribute electric power to the pair of plate-shaped piezoelectric elements.

What is claimed is: 1 . A plate-shaped ultrasonic motor for use in an auto-focus driving part or an OIS driving part of a lens drive apparatus, comprising: a plate-shaped resonant part disposed in parallel to an optical axis; a pair of plate-shaped piezoelectric elements laminated to the plate-shaped resonant part from both sides thereof in a plate-thickness direction; and a single electrode having a pair of plate-shaped terminal parts sandwiching the pair of piezoelectric element from both sides in the plate-thickness direction to distribute electric power to the pair of plate-shaped piezoelectric elements. 2 . A lens drive apparatus comprising: an OIS movable part configured by a first stage and a second stage stacked on a base; the OIS driving part including the plate-shaped ultrasonic motor according to claim 1 and configured to move the OIS movable part in a direction orthogonal to the optical axis; and a driving force transmission part abutting on the plate-shaped ultrasonic motor and configured to convert vibration of the plate-shaped ultrasonic motor to linear motion in the direction orthogonal to the optical axis and transmit a driving force to the OIS movable part. 3 . The lens drive apparatus according to claim 2 , wherein the OIS driving part is supported by a support part disposed to stand at a corner of the base, and the single electrode comprises a contact part connected to a power supply line part provided to the support part to receive power supply. 4 . A camera module, comprising: the lens drive apparatus according to claim 2 ; the lens; and an imaging part configured to take an image of an object formed on the lens. 5 . A camera-mounted apparatus which is an information equipment or a transportation equipment, comprising: the camera module according to claim 4 ; and an image processing part configured to process image information obtained by the camera module. 6 . A lens drive apparatus comprising: a lens holder that accommodates therein a lens and configured movably in a direction of an optical axis direction the auto-focus driving part including the plate-shaped ultrasonic motor according to claim 1 and configured to move the lens holder in the direction of the optical axis; and a driving force transmission part abutting on the plate-shaped ultrasonic motor and configured to convert vibration of the plate-shaped ultrasonic motor to linear motion in the direction of the optical axis and transmit a driving force to the lens holder. 7 . The lens drive apparatus according to claim 6 , further comprising: an OIS movable part having an opening penetrating in the direction of the optical axis, including a first stage and a second stage stacked on a base, and configured to support the lens holder movably in the direction of the optical axis; and the base includes both a power supply line to the auto-focus driving part and a power supply line to the OIS driving part. 8 . A camera module, comprising: the lens drive apparatus according to one of claim 6 ; the lens; and an imaging part configured to take an image of an object formed on the lens. 9 . A camera-mounted apparatus which is an information equipment or a transportation equipment, comprising: the camera module according to claim 8 ; and an image processing part configured to process image information obtained by the camera module.
