Abstract

Official abstract text for this publication.

An energy surgical instrument includes a first grasping piece, a second grasping piece which is allowed to grasp a blood vessel, an energy application unit provided in at least one of the first grasping piece and the second grasping piece, which applies treatment energy to the blood vessel grasped, a setting unit which sets information regarding a wall thickness of the blood vessel, and a grasping force adjustment unit which adjusts a grasping force to a first grasping force when the wall thickness set by the setting unit is larger than a predetermined thickness and adjusting the grasping force to a second grasping force, which is smaller than the first grasping force, when the wall thickness set by the setting unit is equal to or smaller than the predetermined thickness.

First claim

1 . An energy surgical instrument comprising: a first grasping piece; a second grasping piece which is allowed to open and close relative to the first grasping piece and which is allowed to grasp a blood vessel between the first grasping piece and the second grasping piece; an energy application unit provided in at least one of the first grasping piece and the second grasping piece, which applies treatment energy to the blood vessel grasped between the first grasping piece and the second grasping piece to treat the blood vessel; a setting unit which sets whether the blood vessel is a blood vessel of a systemic circulatory system or a blood vessel of a pulmonary circulatory system; and a grasping force adjustment unit which adjusts a grasping force to grasp the blood vessel between the first grasping piece and the second grasping piece to a first grasping force when the setting unit sets the blood vessel to a blood vessel of a systemic circulatory system and adjusting the grasping force to a second grasping force, which is smaller than the first grasping force, when the setting unit sets the blood vessel to a blood vessel of a pulmonary circulatory system. 2 . The energy surgical instrument of claim 1 , further comprising a detector which detects the information regarding the blood vessel, wherein the setting unit sets whether the blood vessel is a blood vessel of a systemic circulatory system or a blood vessel of a pulmonary circulatory system based on a detection result in the detector. 3 - 4 . (canceled) 5 . The energy surgical instrument of claim 2 , wherein: the detector includes a pressure sensor provided in at least one of the first grasping piece and the second grasping piece; and the setting unit determines that the blood vessel is the blood vessel of a pulmonary circulatory system when pressure sensed by the pressure sensor is equal to or higher than a threshold value and determines that the blood vessel is the blood vessel of a systemic circulatory system when the pressure sensed by the pressure sensor is lower than the threshold value. 6 . The energy surgical instrument of claim 2 , wherein: the detector includes a first conductive member provided in the first grasping piece and a second conductive member provided in the second grasping piece and brought into contact with the first conductive member to cause current to flow through an electrical circuit; and the setting unit determines that the blood vessel is the blood vessel of a pulmonary circulatory system based on that current flows through the electrical circuit and determines that the blood vessel is the blood vessel of a systemic circulatory system based on that no current flows through the electrical circuit. 7 . The energy surgical instrument of claim 2 , wherein: the detector includes an angle sensor which senses an angle between the first grasping piece and the second grasping piece; and the setting unit determines that the blood vessel is the blood vessel of a systemic circulatory system when the angle sensed by the angle sensor is equal to or larger than a threshold value and determines that the blood vessel is the blood vessel of a pulmonary circulatory system when the angle sensed by the angle sensor is smaller than the threshold value. 8 . The energy surgical instrument of claim 2 , further comprising a light-emitting element provided in one of the first grasping piece and the second grasping piece, wherein: the detector includes a light-receiving element provided in one of the first grasping piece and the second grasping piece to receive light emitted from the light-emitting element; and the setting unit determines that the blood vessel is the blood vessel of a pulmonary circulatory system when an amount of light received by the light-receiving element is equal to or larger than a threshold value and determines that the blood vessel is the blood vessel of a systemic circulatory system when the amount of light received by the light-receiving element is smaller than the threshold value circuit. 9 . The energy surgical instrument of claim 1 , further comprising an information input unit to which information regarding the blood vessel is input, wherein the setting unit sets whether the blood vessel is a blood vessel of a systemic circulatory system or a blood vessel of a pulmonary circulatory system based on an input result in the information input unit. 10 . The energy surgical instrument of claim 1 , further comprising: a holdable housing; a grip provided in the housing; and a handle revolvably attached to the housing to open or close between the first grasping piece and the second grasping piece when the handle opens and closes relative to the grip, wherein the grasping force adjustment unit includes an abutting member which varies an amount of movement of the handle when the handle closes relative to the grip. 11 . The energy surgical instrument of claim 10 , wherein the abutting member is movable between a first position in which the grasping force is the first grasping force when the handle closes relative to the grip and a second position in which the grasping force is the second grasping force when the handle closes relative to the grip. 12 . The energy surgical instrument of claim 11 , further comprising a selection unit which moves the abutting member to one of the first position and the second position upon receipt of a surgeon's operation. 13 . The energy surgical instrument of claim 11 , further comprising a driving member which moves the abutting member to one of the first position and the second position. 14 . The energy surgical instrument of claim 10 , wherein the abutting member is provided movably to the grip and abuts on the handle when the handle closes relative to the grip, and an amount of protrusion toward the handle from the grip varies when the abutting member moves to the grip. 15 . A surgery method in which a blood vessel is grasped between a first grasping piece and a second grasping piece and energy is applied to the grasped blood vessel to seal the blood vessel, comprising: setting whether the blood vessel is a blood vessel of a systemic circulatory system or a blood vessel of a pulmonary circulatory system; and adjusting a grasping force to grasp the blood vessel between the first grasping piece and the second grasping piece to a first grasping force when the blood vessel is set to a blood vessel of a systemic circulatory system and adjusting the grasping force to a second grasping force, which is smaller than the first grasping force, when the blood vessel is set to a blood vessel of a pulmonary circulatory system.
