Bone plate systems are provided for surgical implants and bone repair. The bone plate is multi-tiered for receiving bone anchors screws for securing a plurality of bones or bone fragments in a desired relationship. The plate members include throughbores for receiving a pivot base therein, with head ends of the bone anchors being secured in the pivot bases. The throughbores may permit and define a translation path for the pivot base and the bone anchor secured therein relative to the plate. Pivot members extending between the pivot base and the plate member facilitate pivoting and optional translation of the pivot base relative to the plate. With the bone anchor seated within the pivot base, an apparatus for inhibiting screw back out is employed.

A method of positioning a pivot base relative to a bone plate, the method comprising: engaging the pivot base and an instrument guide; positioning the pivot base in a desired angular orientation relative to the bone plate via the instrument guide; and advancing a bone penetrating device through a throughbore of the instrument guide and an opening of the pivot base into penetrating engagement with bone. 49 . The method of claim 48 wherein engaging the pivot base comprises resiliently engaging the pivot base with a resilient portion of the instrument guide. 50 . The method of claim 48 including attaching a handle of the instrument guide to a shaft of the instrument guide, and forming a viewing opening down along the shaft between the shaft and the attached handle for viewing the engagement of the pivot base by the instrument guide. 51 . The method of claim 48 wherein the positioning of the pivot base includes axially positioning the pivot base at a desired axial position in an axially elongated throughbore. 52 . A plate holder for positioning a bone plate in a surgical site, the plate holder comprising: an expansion sleeve; a clamping mechanism having a proximal end and a distal end, the clamping mechanism being disposed in the expansion sleeve; and a pair of clamping arms on the clamping mechanism for engaging opposing sides of a bone plate throughbore, the clamping arms expanding apart as the expansion sleeve is shifted toward the distal end of the clamping mechanism and contracting toward one another as the expansion sleeve is shifted toward the proximal end of the clamping mechanism. 53 . The plate holder of claim 52 further comprising: opposing interior surfaces of the clamping arms; an inclined surface on both interior surfaces; and a pin extending through the expansion sleeve and transverse to the travel of the clamping mechanism within the expansion sleeve, the pin ramping along the inclined surfaces to translate longitudinal movement of the expansion sleeve along the clamping mechanism into movement of the clamping arms relative to one another. 54 . A locking guide for positioning a bone plate having a pivot base, the locking guide comprising: a guide tube having engagement arms; a connection arm joined to the guide tube; a compression sleeve disposed coaxially along the guide tube; and a handle rotatably attached to the connection arm and which contacts the compression sleeve during rotation of the handle such that the compression sleeve compresses the engagement arms of the guide tube toward one another to engage the pivot base when the handle is rotated toward the connection arm. 55 . The method of claim 48 wherein advancing the bone penetrating device through the throughbore of the instrument guide and the opening of the pivot base into penetrating engagement with bone includes advancing a bone screw through the throughbore of the instrument guide and the opening of the pivot base and driving the bone screw into the bone. 56 . The method of claim 48 wherein engaging the pivot base includes engaging members of the instrument guide with opposite sides of the pivot base. 57 . The method of claim 56 wherein engaging members of the instrument guide with the opposite sides of the pivot base includes positioning the members in recesses of the pivot base. 58 . The method of claim 48 wherein engaging the pivot base includes advancing a portion of the instrument guide into a gap between the pivot base and the bone plate. 59 . The method of claim 48 wherein engaging the pivot base includes advancing an end of the instrument guide into gaps between the pivot base and the bone plate on opposite sides of the pivot base. 60 . The method of claim 48 wherein engaging the pivot base includes advancing a portion of one of the instrument guide and the pivot base at least partially into a recess of the other of the instrument guide and the pivot base. 61 . The method of claim 48 wherein engaging the pivot base includes engaging teeth of the instrument guide in indentations on opposite sides of the pivot base. 62 . The method of claim 48 wherein engaging the pivot base includes positioning a distal surface of the instrument guide against an upper surface of the pivot base. 63 . The method of claim 48 wherein positioning the pivot base includes pivoting the pivot base relative to the bone plate via the instrument guide. 64 . The method of claim 48 wherein positioning the pivot base includes using the instrument guide to pivot the pivot base about a pivot axis defined by a pin connecting the pivot base to the bone plate. 65 . The method of claim 48 wherein the bone plate is elongated along a bone plate axis and positioning the pivot base includes pivoting the pivot base using the guide instrument guide about an axis extending orthogonal to the bone plate axis. 66 . The method of claim 48 further comprising advancing a bone preparation tool through the throughbore of the instrument guide to prepare the bone prior to advancing the bone penetrating device through the throughbore of the instrument guide. 67 . The method of claim 48 wherein engaging the pivot base includes compressing arms of the instrument guide against the pivot base.
