1 - 11 . (canceled) 12 . A bone anchoring device for coupling a rod to a bone anchoring element, the bone anchoring device comprising: a receiving part having a first end configured to pivotably receive a head of the bone anchoring element, the receiving part further having a recess with a substantially U-shaped cross-section for receiving the rod, the recess defining two legs, the legs having an internal advancement structure; a pressure element configured to contact the head to exert pressure on the head to lock an angular position of the head relative to the receiving part; and a locking assembly comprising: a first locking element having a longitudinal axis, an outer surface with an external advancement structure configured to cooperate with the internal advancement structure of the legs, an inner wall defining a coaxial bore extending through the first locking element along the longitudinal axis, and an internal advancement structure on a portion of the inner wall; and a second locking element configured to be arranged at least partially in the first locking element, the second locking element having an outer surface with an external advancement structure configured to cooperate with the internal advancement structure of the first locking element; wherein when the locking assembly is in the receiving part and the first locking element is advanced axially in the receiving part at a first speed, the second locking element is configured to move axially relative to the receiving part at a speed different than the first speed, such that the second locking member urges the pressure element towards the first end of the receiving part to lock an inserted head relative to the receiving part and the first locking element locks an inserted rod relative to the receiving part. 13 . The bone anchoring device of claim 12 , wherein the respective advancement structures of the first locking element and the second locking element comprise threads, and wherein the greatest width of the external thread of the second locking element is a major diameter of the thread. 14 . The bone anchoring device of claim 13 , wherein the external thread of the first locking element has a first thread pitch, and the internal thread of the first locking element has a second thread pitch that is smaller than the first thread pitch. 15 . The bone anchoring device of claim 12 , wherein the second locking element comprises a first end, a second end, and an engagement portion at the second end that extends away from the first end, wherein when the pressure element, the rod, and the locking assembly are inserted in the receiving part, the engagement portion is configured to contact the pressure element without contacting a portion of the rod positioned closest to the first locking element. 16 . The bone anchoring device of claim 15 , wherein the engagement portion comprises two spaced apart projections configured to contact the pressure element. 17 . The bone anchoring device of claim 15 , wherein the portion of the second locking element comprises projecting edges projecting from the engagement portion in a direction perpendicular to a direction of movement of the second locking element relative to the receiving part. 18 . The bone anchoring device of claim 12 , wherein the first and second locking elements are configured to move together in a first direction relative to the receiving part when the locking assembly is advanced axially in the receiving part. 19 . (canceled) 20 . The bone anchoring device of claim 12 , wherein movement of the second locking element relative to the first locking element causes an inserted rod to be locked relative to the receiving part sequentially after an inserted head is locked relative to the receiving part. 21 . (canceled) 22 . The bone anchoring device of claim 12 , wherein when the locking assembly is advanced in a first axial direction relative to the receiving part, the second locking element is configured to move in a direction opposite the first axial direction relative to the first locking element. 23 - 29 . (canceled) 30 . The bone anchoring device of claim 13 , wherein the internal thread of the first locking element and the external thread of the first locking element are oriented in opposite rotational directions. 31 . The bone anchoring device of claim 13 , wherein the internal thread of the first locking element is a flat thread. 32 . The bone anchoring device of claim 12 , wherein the first locking element and the second locking element are preassembled in such a way that the second locking element is rotationally fixed with respect to the first locking element in a temporary manner. 33 . The bone anchoring device of claim 32 , wherein the temporary fixation is formed by a connection between the first and second locking elements that is separable by rotating the first locking element relative to the second locking element. 34 . The bone anchoring device of claim 33 , wherein the second locking element is prevented from rotating relative to the receiving part, such that when the first locking element is rotated relative to the receiving part, the connection between the first and second locking elements is separated. 35 . The bone anchoring device of claim 12 , wherein the second locking element comprises a portion configured to prevent the second locking element from rotating relative to the receiving part when the locking assembly is inserted in to the receiving part, wherein the portion projects in a direction perpendicular to a direction of movement of the second locking element relative to the receiving part. 36 . The bone anchoring device of claim 12 , wherein the respective advancement structures of the first locking element and the second locking element comprise threads, wherein the first locking element moves axially relative to the receiving part when a rotational orientation of the first locking element is adjusted, and wherein the second locking element is prevented from rotating relative to the receiving part. 37 . The bone anchoring device of claim 36 , wherein the first locking element assumes a first rotational orientation when the locking assembly is at a first position relative to the receiving part where an inserted head and an inserted rod are locked relative to the receiving part, and wherein the first locking element is rotatable to a second rotational orientation where the first locking element is displaced axially by an amount greater than an axial displacement of the second locking element, such that the inserted rod is unlocked while the inserted head remains locked. 38 . A bone anchoring device for coupling a rod to a bone anchoring element, the bone anchoring device comprising: a receiving part configured to pivotably receive a head of the bone anchoring element, and having a recess with a substantially U-shaped cross-section for receiving the rod, the recess defining two legs, the legs having an internal thread with a first thread pitch; and a locking assembly comprising: a first locking element having a longitudinal axis, an outer surface with an external thread configured to cooperate with the internal thread of the legs, an inner wall defining a coaxial bore extending through the first locking element along the longitudinal axis, and an internal thread with a second thread pitch different from the first thread pitch on a portion of the inner wall; and a second locking element configured to be arranged at least partial
