This disclosure describes systems, devices, and techniques for automatically identifying missing or ambiguous information in documentation associated with a patient. In one example, a computerized system for updating medical documentation may include one or more computing devices configured to receive a code representative of one or more undocumented items determined from a plurality of documented items related to the patient. The one or more computing devices may be configured to generate, based on the code, query that solicits user input addressing the one or more undocumented items and output, for display, the query.

What is claimed is: 1 . A computer-implemented method for requesting medical information associated with a patient, the method comprising: receiving, by a computing device, a code representative of one or more undocumented items determined from a plurality of documented items related to the patient; generating, by the computing device and based on the code, a query that solicits user input addressing the one or more undocumented items; and outputting, by the computing device and for display, the query. 2 . The method of claim 1 , further comprising presenting, via a display device, a user interface comprising the query. 3 . The method of claim 2 , wherein the query comprises one or more selectable items that, when selected, addresses the one or more undocumented items. 4 . The method of claim 3 , wherein at least one of the one or more selectable items corresponds to a respective one of the one or more undocumented items, and wherein selection of one of the selectable items addresses the one or more undocumented items represented by the code. 5 . The method of claim 4 , wherein one of the one or more selectable items comprises a rejection input that rejects all of the one or more selectable items corresponding to the respective one of the one or more undocumented items, and wherein selection of the rejection input addresses the one or more undocumented items represented by the code. 6 . The method of claim 2 , wherein the query comprises one of a question or a statement, and wherein the method further comprises receiving, via the user interface, a query response that is responsive to either the question or the statement. 7 . The method of claim 6 , further comprising updating, based on the query response to the presented query, documentation associated with the patient to define one or more medical concepts. 8 . The method of claim 2 , further comprising presenting, via the user interface, the query with a problem list associated with the patient, and wherein user input solicited by the query triggers an update to the problem list. 9 . The method of claim 1 , wherein the query is one of a plurality of queries associated with one or more respective undocumented items, and wherein the method further comprises: generating, for display in association with the patient, indicia representative of a number of the plurality of queries requiring a respective response. 10 . The method of claim 1 , further comprising: generating, based on at least a portion of encounter-related information associated with the patient, medical evidence associated with the patient; and outputting, for display with the query, at least a portion of the medical evidence. 11 . The method of claim 1 , wherein the user is a physician. 12 . A computerized system for requesting medical documentation associated with a patient, the system comprising one or more computing devices configured to: receive a code representative of one or more undocumented items determined from a plurality of documented items related to the patient; generate, based on the code, an query that solicits user input addressing the one or more undocumented items; and output, for display, the query. 13 . The system of claim 12 , wherein the one or more computing devices are configured to present, via a display device, a user interface comprising the query. 14 . The system of claim 13 , wherein the query comprises one or more selectable items that, when selected, addresses the one or more undocumented items. 15 . The system of claim 14 , wherein at least one of the one or more selectable items corresponds to a respective one of the one or more undocumented items, and wherein selection of one of the selectable items addresses the one or more undocumented items represented by the code. 16 . The system of claim 15 , wherein one of the one or more selectable items comprises a rejection input that rejects all of the one or more selectable items corresponding to the respective one of the one or more undocumented items, and wherein selection of the rejection input addresses the one or more undocumented items represented by the code. 17 . The system of claim 13 , wherein the query comprises one of a question or a statement, and wherein the method further comprises receiving, via the user interface, a query response that is responsive to either the question or the statement. 18 . The system of claim 17 , wherein the one or more computing devices are configured to update, based on the query response to the presented query, documentation associated with the patient to define one or more medical concepts. 19 . The system of claim 13 , wherein the one or more computing devices are configured to present, via the user interface, the query with a problem list associated with the patient, and wherein user input solicited by the query triggers an update to the problem list. 20 . The system of claim 12 , wherein the query is one of a plurality of queries associated with one or more respective undocumented items, and wherein the one or more computing devices are configured to: generate, for display in association with the patient, indicia representative of a number of the plurality of queries requiring a respective response. 21 . The system of claim 12 , wherein the one or more computing devices are configured to: generate, based on at least a portion of encounter-related information associated with the patient, medical evidence associated with the patient; and output, for display with the query, at least a portion of the medical evidence. 22 . The system of claim 12 , wherein the one or more computing devices comprise: a query generation module configured to: receive the code representative of the one or more undocumented items; and generate, based on the code, the query that solicits user input addressing the one or more undocumented items. 23 . The system of claim 12 , wherein the user is a physician. 24 . A computer-readable storage medium comprising instructions that, when executed, cause one or more processors to: receive a code representative of one or more undocumented items determined from a plurality of documented items related to the patient; generate, based on the code, an query that solicits user input addressing the one or more undocumented items; and output, for display, the query. 25 . The computer-readable storage medium of claim 24 , further comprising instructions that cause the one or more processors to present, via a display device, a user interface comprising the query. 26 . The computer-readable storage medium of claim 25 , wherein the query comprises one of a question or a statement, and wherein the computer-readable storage medium comprises instructions that cause one or more processors to receive, via the user interface, a query response that is responsive to either the question or the statement.
