Abstract

Official abstract text for this publication.

System and methods for channeling a path into bone include a trocar having a proximal end, distal end and a central channel disposed along a central axis of the trocar. The trocar includes a distal opening at the distal end of the trocar. The system includes a curved cannula sized to be received in the central channel, the curved cannula comprising a curved distal end configured to be extended outward from the distal opening to generate a curved path extending away from the trocar. The curved cannula has a central passageway having a diameter configured to allow a treatment device to be delivered through the central passageway to a location beyond the curved path.

What is claimed is: 1 . A method for ablation of a basivertebral nerve, the method comprising: inserting a trocar into a vertebra; inserting a cannula arrangement and a stylet arrangement into a central lumen of the trocar, wherein the cannula arrangement defines a cannula lumen and comprises a proximal end portion and a distal portion, wherein the distal portion of the cannula arrangement is configured to be curved and is configured to be straightened, wherein the proximal end portion comprises a first alignment member, wherein the stylet arrangement comprises a handle and a distal portion, wherein the distal portion of the stylet arrangement is configured to be curved and is configured to be straightened, wherein the handle comprises a second alignment member, forming a curved pathway through a cancellous portion of the vertebra by advancing the cannula arrangement and the stylet arrangement past an opening at a distal tip of the trocar and positioning the distal portion of the stylet arrangement at a target area proximal to a junction of the basivertebral nerve of the vertebra, wherein the first alignment member of the cannula arrangement is aligned and engaged with the second alignment member of the stylet arrangement in a predefined configuration and a distal tip of the distal portion of the stylet arrangement protrudes beyond the distal portion of the cannula arrangement; removing the stylet arrangement; inserting a bipolar radiofrequency (RF) probe into the cannula lumen of the cannula arrangement and delivering a distal end portion of the bipolar RF probe through the curved pathway to a position immediately adjacent to the basivertebral nerve in the target area; and ablating at least a portion of the basivertebral nerve using the bipolar RF probe. 2 . The method of claim 1 , further comprising, after the ablating, removing the bipolar RF probe from the cannula arrangement. 3 . The method of claim 1 , wherein engagement of the first and second alignment members prevents rotation of the stylet arrangement with respect to the cannula arrangement. 4 . The method of claim 1 , wherein a one of the first alignment member or the second alignment member comprises a notch. 5 . The method of claim 1 , wherein the first alignment member comprises a key and the second alignment member comprises a groove which interfaces with the key. 6 . The method of claim 1 , wherein the distal portion of the cannula arrangement comprises a compliant material. 7 . The method of claim 1 , wherein the distal portion of the cannula arrangement comprises a polymer tube. 8 . The method of claim 7 , wherein the polymer tube is made of a rigid polymer. 9 . The method of claim 1 , further comprising inserting the stylet arrangement into the cannula lumen of the cannula arrangement; and aligning and engaging the first alignment member of the cannula arrangement with the second alignment member of the stylet arrangement in the predefined configuration. 10 . The method of claim 9 , wherein the distal portion of the stylet arrangement, prior to insertion into the cannula arrangement, has a curved configuration and a straight configuration. 11 . The method of claim 10 , wherein inserting the stylet arrangement further comprises inserting the stylet arrangement into the cannula lumen with the stylet arrangement in the straight configuration. 12 . The method of claim 10 , further comprising, with the stylet arrangement at least partially inserted into the cannula arrangement, changing the stylet arrangement from the straight configuration to the curved configuration. 13 . The method of claim 9 , wherein inserting the stylet arrangement into the cannula lumen of the cannula arrangement occurs prior to inserting the cannula arrangement and the stylet arrangement into the central lumen of the trocar. 14 . The method of claim 1 , wherein inserting the cannula arrangement and the stylet arrangement comprises inserting the cannula arrangement into the central lumen of the trocar and then inserting the stylet arrangement into the cannula lumen of the cannula arrangement. 15 . A method for ablation of a basivertebral nerve, the method comprising: inserting a trocar into a vertebra and forming a straight pathway in a bone region; inserting a curveable arrangement into a central lumen of the trocar, wherein the curveable arrangement includes a cord or wire directly attached to a distal portion of the curveable arrangement, wherein the cord or wire is attached to a curvature control, wherein operation of the curvature control pulls the cord or wire, relative to the curveable arrangement, to alter tension of the cord or wire to modify a curvature of the distal portion of the curveable arrangement when the distal portion of the curveable arrangement extends out of the trocar; with at least a portion of the distal portion of the curveable arrangement extending out of the trocar, producing a curved pathway through a cancellous portion of the vertebra beyond the trocar and toward the basivertebral nerve to position the distal portion of the curveable arrangement at a target area proximal to a junction of the basivertebral nerve of the vertebra, wherein the curved pathway is curved relative to the straight pathway formed by the trocar, wherein, to produce the curved pathway, the curveable arrangement includes a curveable cannula defining a cannula lumen, a proximal body attached to a proximal end of the curveable cannula, an elongate tube configured for insertion into the cannula lumen and extension out of a distal end of the curveable cannula to produce a portion of the curved pathway through the cancellous portion of the vertebra beyond the distal end of the curveable cannula, and a handle attached to a proximal end of the elongate tube, wherein the proximal body comprises a first alignment member and the handle comprises a second alignment member, wherein inserting the curveable arrangement comprises aligning and engaging the first alignment member with the second alignment member in a predefined configuration to restrict rotation of the elongate tube relative to the curveable cannula, wherein producing the curved pathway comprises extending a distal tip of the elongate tube out of the distal end of the curveable cannula; delivering a plurality of electrodes through the curveable cannula and along the curved pathway to position the electrodes immediately adjacent to the basivertebral nerve in the target area; coupling the electrodes to a power source; and ablating at least a portion of the basivertebral nerve using the electrodes. 16 . The method of claim 15 , further comprising removing the electrodes and curveable arrangement after the ablating. 17 . The method of claim 15 , wherein the first alignment member comprises a key and the second alignment member comprises a groove which interfaces with the key. 18 . The method of claim 15 , wherein a part of the distal portion of the curveable arrangement comprises a plurality of mating links. 19 . The method of claim 18 , wherein the mating links comprise interlocking surfaces. 20 . The method of claim 19 , wherein the interlocking surfaces are arranged to allow the distal portion of the curveable arrangement to curve in one direction. 21 . The method of claim 18 , wherein, for each of a plurality of adjacent pairs of the mating links, a first one of the mating links of the adjacent pair comprises an extension and a second one of the mating links of the
