An anchor for being embedded in concrete for attachment to a fastener to support a load comprises a plug having a main body portion extending upwardly from a base portion, the plug for being attached to a form board prior to pouring of concrete, the plug having an end portion disposed a distance from the form board; and an anchor body attached to the end portion. The plug is separable from the anchor body and removable from the concrete after the concrete is cured, leaving the anchor body embedded in the concrete, the plug providing a void in the concrete after removal to provide an access opening for a threaded portion of a fastener to attach to the anchor body.

I claim: 1 . A plug, comprising: a) a body for attachment to a form board prior to pouring of concrete, the body including an end portion disposed a distance from the form board; b) a helical member disposed at the end portion; c) the body for being removable from the concrete after the concrete is cured and separable from the helical member to leave the helical member in the concrete, the body for providing a void in the concrete after removal from the concrete to provide an access opening for a a fastener for attaching to the helical member in the concrete. 2 . The plug as in claim 1 , wherein the body has a columnar main body portion. 3 . The plug as in claim 2 , wherein: a) the body includes a base portion; and b) the main body portion extends from the base portion. 4 . The plug as in claim 3 , wherein the main body portion and the base portion are tapered. 5 . The plug as in claim 1 , wherein the end portion is threaded with a single revolution thread. 6 . The plug as in claim 1 , wherein a) the end portion includes threads; and b) the helical member is a wire coil threaded around the threads of the end portion. 7 . The plug as in claim 6 , wherein the wire coil extends past a top of the end portion. 8 . The plug as in claim 6 , wherein the wire coil is made from shaped metal with a profile corresponding to a profile of the threads. 9 . The plug as in claim 1 , wherein: a) the end portion includes a cylindrical portion; and b) the helical member includes a wire coil attached to the cylindrical portion. 10 . The plug as in claim 1 , wherein: a) the end portion includes a lower portion, an upper portion and a transition portion between the lower portion and the upper portion, the lower portion has a larger diameter than the upper portion; and b) the helical member includes a wire coil conforming to a diameter of the lower portion, the transition portion and a diameter of the upper portion. 11 . The plug as in claim 10 , wherein the wire coil is threaded to the lower portion. 12 . The plug as in claim 1 , wherein: a) the end portion includes a threaded portion; and b) the helical member includes a metal sleeve with a wall formed with internal and external threads conforming to the threaded portion. 13 . The plug as in claim 12 , wherein the metal sleeve is closed at a top end. 14 . The plug as in claim 1 , wherein: a) the end portion includes a single revolution thread; and b) the helical member is an inverted cup formed with a side wall formed with internal and external threads conforming to the single revolution thread. 15 . The plug as in claim 1 , wherein: a) the end portion includes tapered threads; and b) the helical member is a tapered metal sleeve formed with internal and external threads conforming to the tapered threads. 16 . The plug as in claim 1 , wherein: a) the end portion includes threads; and b) the helical member includes a non-stick tape wrapped around the end portion. 17 . The plug as in claim 1 , wherein: a) the end portion includes threads; and b) the helical member includes a metal foil wrapped around the end portion. 18 . A plug, comprising: a) a body for attachment to a form board prior to pouring of concrete, the body including an end portion disposed a distance from the form board; b) the end portion including a first circumferential groove and a first ring partly disposed in the groove, the first ring for molding a second circular groove in the concrete; c) the body and the first ring for being removable from the concrete after the concrete is cured for leaving a mold of the first ring in the concrete, the body for providing a void in the concrete after removal to provide an access opening to the second circular groove for attachment of a fastener. 19 . A plug, comprising: a) a body for being attached to a form board with screws and/or nails prior to pouring of concrete, the body including an end portion disposed a distance from the form board, the end portion for positioning an anchor body in the concrete; b) the body being configured to allow the screws and/or nails to remain attached to the form board when the form board is removed; and c) the body providing an opening through the concrete for access to the anchor body after the form board is removed after the concrete is cured.
