An implant for use in orthognathic surgery of a mandible may include a longitudinal plate member and a plurality of pre-configured guides coupled to the plate member. The longitudinal plate member is pre-bent to correspond to the post-operative shape of the mandible; and the guides are pre-configured to align the plate member with the mandible when the implant is positioned against the mandible after the mandible has been separated.

What is claimed: 1. A method of repositioning performing an osteotomy on a maxilla, the method comprising: attaching an osteotomy guiding implant to a first portion of the maxilla and a second portion of the maxilla while the maxilla is in a pre-operative shape whereby the first and second portions define a first relative position, after the attaching step, cutting the maxilla so as to separate the first portion from the second portion; after the cutting step, moving the first portion of the maxilla with respect to the second portion of the maxilla to achieve a post-operative shape of the maxilla whereby the first and second portions define a second relative position that is different than the first relative position; and after the moving step, securing the second portion of the maxilla to the first portion of the maxilla with a bone fixation implant that is pre-operatively shaped to correspond to the post-operative shape of the maxilla, wherein the guiding implant is pre-operatively shaped to correspond to the pre-operative shape of the maxilla. 2. The method of claim 1 , the method further comprising placing the guiding implant against the maxilla such that a bone engaging surface of the guiding implant lies substantially flush with the maxilla prior to the attaching step. 3. The method of claim 2 , wherein the bone engaging surface comprises a plurality of non-linear undulations that correspond to surface portions of the maxilla after the attaching step. 4. The method of claim 2 , wherein the placing step comprises placing a plate portion of the guiding implant against the first portion of the maxilla, and placing at least one finger of the guiding implant against the second portion of the maxilla, wherein the at least one finger defines an osteotomy cutting guide. 5. The method of claim 1 , wherein the attaching step comprises attaching a plate portion of the guiding implant against the first portion of the maxilla, and attaching at least one finger of the guiding implant against the second portion of the maxilla. 6. The method of claim 5 , wherein the attaching step comprises driving at least one bone screw through the plate portion and into the first portion of the maxilla. 7. The method of claim 6 , further comprising the step of removing the guiding implant from the maxilla prior to the securing step, wherein the removing step reveals a screw hole that extends into the maxilla. 8. The method of claim 7 , wherein the securing step comprises driving a bone screw into the screw hole so as to secure the bone fixation implant to the maxilla. 9. The method of claim 1 , wherein the osteotomy guiding implant comprises a plurality of apertures disposed along an osteotomy cutting path, and the cutting step is along the osteotomy cutting path.
