An accommodating intraocular lens (AIOL) for implantation within a capsular bag of a patient's eye comprises first and second components coupled together to define an inner fluid chamber and an outer fluid reservoir. The inner region of the AIOL provides optical power with one or more of the shaped fluid within the inner fluid chamber or the shape of the first or second components. The fluid reservoir comprises a bellows region with fold(s) extending circumferentially around an optical axis of the eye. The bellows engages the lens capsule, and a compliant fold region between inner and outer bellows portions allows the profile of the AIOL to deflect when the eye accommodates for near vision. Fluid transfers between the inner fluid chamber and the outer fluid reservoir to provide optical power changes. A third lens component coupled to the first or second component provides additional optical power.

First claim

The invention claimed is: 1. An accommodating intraocular lens assembly comprising: a base having an accommodating lens with an optical axis, the accommodating lens configured to change optical power along the optical axis in reaction to radial force on the base in a direction non-parallel to the optical axis; and an exchangeable non-accommodating lens configured to removably couple to the base, the exchangeable non-accommodating lens positioned anterior to and spaced apart from the accommodating lens when the exchangeable non-accommodating lens is coupled to the base. 2. The assembly of claim 1 wherein the exchangeable non-accommodating lens is aligned with the accommodating lens along the optical axis when the exchangeable non-accommodating lens is coupled to the base. 3. The assembly of claim 1 wherein a thickness of the accommodating lens, in a direction parallel to the optical axis, changes in reaction to radial force on the base in a direction non-parallel to the optical axis. 4. The assembly of claim 1 wherein the base includes a groove configured to receive at least a portion of the exchangeable non-accommodating lens when the exchangeable non-accommodating lens is coupled to the base. 5. The assembly of claim 1 wherein the base includes a haptic connected to the accommodating lens, the haptic configured to receive a radial force that causes the accommodating lens to change optical power. 6. The assembly of claim 5 wherein the haptic is an annular bellows configured to push fluid into the accommodating lens when radially-compressive force is applied to the bellows. 7. The assembly of claim 1 wherein the accommodating lens includes a radially-outer wall having one or more openings therein. 8. The assembly of claim 7 wherein the base includes a haptic reservoir in fluid communication with the accommodating lens via the one or more openings. 9. The assembly of claim 1 wherein a width of the accommodating lens, as measured perpendicular to the optical axis, remains constant as the accommodating lens changes optical power. 10. The assembly of claim 1 wherein the exchangeable non-accommodating lens is configured to couple with and decouple from the base when the base is implanted in a native eye capsule of an eye of an individual. 11. An accommodating intraocular lens assembly comprising: a base having a fluid lens with an optical axis, the fluid lens configured to change optical power along the optical axis in reaction to radial force on the base in a direction non-parallel to the optical axis; and an exchangeable non-accommodating lens configured to removably couple to the base, the exchangeable non-accommodating lens positioned anterior to the fluid lens when the exchangeable non-accommodating lens is coupled to the base. 12. The assembly of claim 11 wherein the base includes an annular haptic structure connected to the fluid lens, wherein the annular haptic structure includes a fluid reservoir in fluid communication with the fluid lens, and wherein the annular haptic structure is configured to transfer fluid into the fluid lens when the radial force is applied to the base. 13. The assembly of claim 12 wherein the base includes an annular wall between the fluid lens and the annular haptic structure, and wherein the annular wall includes one or more openings configured to permit fluid to pass between the fluid reservoir and the fluid lens. 14. The assembly of claim 11 wherein the fluid lens comprises a silicon oil. 15. The assembly of claim 11 wherein a volume of the fluid lens increases in reaction to the radial force on the base. 16. The assembly of claim 11 wherein a thickness of the exchangeable non-accommodating lens, as measured parallel to the optical axis, remains constant when the radial force is applied to the base. 17. An accommodating intraocular lens assembly comprising: a base having a first optical component and a second optical component spaced from the first optical component along an optical axis of the base, the first and second optical components configured to move with respect to each other along the optical axis in reaction to radial force on the base in a direction non-parallel to the optical axis; and an exchangeable fixed power lens configured to removably couple to the base, the exchangeable fixed power lens positioned anterior to the base when the exchangeable fixed power lens is coupled to the base. 18. The assembly of claim 17 wherein the base includes an annular outer wall at an outer periphery of the first and second optical components, wherein the outer wall is formed by one or both of the first and second optical components, and wherein the outer wall includes one or more openings therein. 19. The assembly of claim 18 wherein the base includes a haptic volume at least partially surrounding one or both of the first and second optical components, and wherein the haptic volume is in fluid communication with a space between the first and second optical components. 20. The assembly of claim 17 wherein the base includes an optical chamber between the first and second optical components, the optical chamber configured to change shape when the first and second optical components move with respect to each other.
