A user device can be used to generate medical term expressions, which represent medical terms of a health record. The user device can identify a medical concept present in the health record based on a medical term expression. The user device can generate a node in a personalized relational graph that corresponds to the medical concept. One or more sub-nodes can be added to the node. Responsive to a request, a user interface is presented that identifies the medical concept and some of the additional information.

What is claimed is: 1. A computer-implemented method, comprising: downloading, to a user device and from an electronic health record system, a data package that includes a health record; generating, by the user device, a plurality of medical term expressions that represent a plurality of medical terms from an entry of the health record downloaded to the user device; for the entry of the health record, identifying, by the user device, a medical concept present in the entry based at least in part on at least one medical term expression of the plurality of medical term expressions; generating, by the user device, a node in a personalized relational graph that corresponds to the medical concept and identifies the medical term expression, wherein the personalized relational graph represents an abstraction of the health record and other health records downloaded to the user device; adding, by the user device, one or more sub-nodes to the node in the personalized relational graph based at least in part on additional information relating to the medical concept; and responsive to a request, providing, by the user device and for presentation at the user device, a user interface view that identifies the medical concept and presents at least a portion of the additional information. 2. The computer-implemented method of claim 1 , further comprising: downloading a different data package that includes a different health record from a different electronic health record system; and generating a different node in the personalized relational graph that corresponds to a different medical concept represented in the different health record. 3. The computer-implemented method of claim 1 , wherein the additional information comprises first information sourced from the health record and second information obtained from a remote system, and wherein the second information is associated with the first information via the node. 4. The computer-implemented method of claim 1 , wherein generating the plurality of medical term expressions comprises: storing the health record using a first data model; indexing the plurality of medical terms; and storing the plurality of medical terms using a second data model that is distinct from the first data model, wherein the second data model represents the personalized relational graph. 5. The computer-implemented method of claim 1 , wherein the user interface view identifies a plurality of medical concepts that includes the medical concept, and wherein each medical concept of the plurality of medical concepts is represented by at least one node in the personalized relational graph. 6. The computer-implemented method of claim 1 , wherein the health record is maintained by the electronic health record system. 7. The computer-implemented method of claim 6 , wherein the personalized relational graph is specific to a user of the user device and represents health information of the user sourced from the health record and sourced from other health records maintained by other electronic health record systems. 8. One or more non-transitory computer-readable media comprising computer-executable instructions that, when executed by a processor of a user device, causes the user device to perform operations, comprising: downloading, to the user device and from an electronic health record system, a data package that includes a health record; generating, by the user device, a plurality of medical term expressions that represent a plurality of medical terms from an entry of the health record downloaded to the user device; for the entry of the health record, identifying, by the user device, a medical concept present in the entry based at least in part on at least one medical term expression of the plurality of medical term expressions; generating, by the user device, a node in a personalized relational graph that corresponds to the medical concept and identifies the medical term expression, wherein the personalized relational graph represents an abstraction of the health record and other health records downloaded to the user device; adding, by the user device, one or more sub-nodes to the node in the personalized relational graph based at least in part on additional information relating to the medical concept; and responsive to a request, providing, by the user device and for presentation at the user device, a user interface view that identifies the medical concept and presents at least a portion of the additional information. 9. The one or more non-transitory computer-readable media of claim 8 , wherein the computer-readable media comprise additional computer-executable instructions that, when executed by the processor of the user device, cause the user device to perform additional operations comprising: downloading a different data package that includes a different health record from a different electronic health record system; and generating a different node in the personalized relational graph that corresponds to a different medical concept represented in the different health record. 10. The one or more non-transitory computer-readable media of claim 8 , wherein the additional information comprises first information sourced from the health record and second information obtained from a remote system, and wherein the second information is associated with the first information via the node. 11. The one or more non-transitory computer-readable media of claim 8 , wherein generating the plurality of medical term expressions comprises: storing the health record using a first data model; indexing the plurality of medical terms; and storing the plurality of medical terms using a second data model that is distinct from the first data model, wherein the second data model represents the personalized relational graph. 12. The one or more non-transitory computer-readable media of claim 8 , wherein the user interface view identifies a plurality of medical concepts that includes the medical concept, and wherein each medical concept of the plurality of medical concepts is represented by at least one node in the personalized relational graph. 13. The one or more non-transitory computer-readable media of claim 8 , wherein the health record is maintained by the electronic health record system. 14. The one or more non-transitory computer-readable media of claim 13 , wherein the personalized relational graph is specific to a user of the user device and represents health information of the user sourced from the health record and sourced from other health records maintained by other electronic health record systems. 15. A user device, comprising: a memory configured to store computer-executable instructions; and a processor configured to access the memory and execute the computer-executable instructions to at least: download, to the user device and from an electronic health record system, a data package that includes a health record; generate, by the user device, a plurality of medical term expressions that represent a plurality of medical terms from an entry of the health record downloaded to the user device; for the entry of the health record, identify, by the user device, a medical concept present in the entry based at least in part on at least one medical term expression of the plurality of medical term expressions; generate, by the user device, a node in a personalized relational graph that corresponds to the medical concept and identifies the medical term expression, wherein the personalized relational graph represents an abstraction of the health record and other health records downloaded to the user device; a
