The present invention provides a method of testing a blood for a macrolide immunosuppressant conveniently and/or highly accurately. Specifically, the present invention provides a method of testing a blood for a macrolide immunosuppressant, including: (a) treating a blood sample containing a macrolide immunosuppressant or a metabolite thereof having a macrolide structure with an acid or an alkali; and (b) measuring a concentration of the macrolide immunosuppressant or the metabolite in the blood sample by using an antibody.

The invention claimed is: 1. A method of testing a blood for a macrolide immunosuppressant that is tacrolimus, the method comprising: (a) treating a blood sample comprising tacrolimus with an acidic buffer having a pH in a range of 2.0 to 4.5 to form an acidic blood sample containing the tacrolimus, and wherein the acidic blood sample containing the tacrolimus does not contain a volatile organic solvent; and (b) measuring a concentration of the tacrolimus in the acidic blood sample by contacting the acidic blood sample with an antibody that specifically binds to the tacrolimus, and the antibody is IgG, IgM, or an antibody fragment thereof, and immobilized to a solid phase. 2. The method of claim 1 , wherein the blood sample is a human blood sample.
