Patient-specific guides for the Latarjet procedure, as well as surgical systems and methods of performing the Latarjet procedure to treat glenohumeral instability using such patient-specific guides are disclosed. A patient-specific coracoid guide and a patient-specific glenoid guide may be configured based on preoperatively generated three-dimensional models of the patient's shoulder anatomy. Guides may be configured for coracoid graft preparation and glenoid decortication. The coracoid graft may be placed in the desired position based on three-dimensional (3D) preoperative planning.

What is claimed is: 1. A coracoid guide comprising: an osteotomy cutting slot configured to receive a cutting element therethrough to guide removal of a coracoid process from a scapula of a specific patient; a medial cutting slot configured to receive the cutting element therethrough to guide preparation of a medial surface of the removed coracoid process; and one or more guide holes adapted to guide the creation of one or more holes in the coracoid process; wherein the coracoid guide defines a first surface configured to conform to a surface of the coracoid process of the specific patient. 2. A glenoid guide comprising: a decortication cutting slot configured to receive a cutting element therethrough to guide decortication of a margin of a glenoid of a specific patient; and one or more guide holes adapted to guide the creation of one or more holes in the decorticated margin of the glenoid of the specific patient; wherein the glenoid guide defines a first surface configured to conform to a surface of the glenoid of the specific patient. 3. A system including a graft positioning guide for performing a Latarjet procedure, comprising: a first patient-matched portion having a first surface configured to conform to a glenoid surface of a specific patient and configured to receive the glenoid surface; a second patient-matched portion having a second surface configured to conform to a surface of a coracoid graft and configured to receive the coracoid graft of the specific patient; and one or more holes defined in the graft positioning guide adapted for positioning one or more fasteners to bind the coracoid graft to a prepared portion of the glenoid surface of the specific patient; and a patient-specific coracoid guide defining a surface configured to conform to a coracoid process of a specific patient and a patient-specific glenoid guide defining a surface configured to confirm to the glenoid of the specific patient are adapted to be used to prepare the coracoid process of the specific patient for attachment to the glenoid of the specific patient in a preoperatively planned position; and wherein the graft positioning guide is configured to hold the coracoid process in the preoperatively planned position with respect to the glenoid while fasteners are placed. 4. A set of patient-specific guides for performing a Latarjet procedure comprising: a graft positioning guide comprising: a first patient-matched portion having a first surface configured to conform to a glenoid surface of a specific patient; and a second patient-matched portion having a second surface configured to conform to a surface of a corticoid graft removed from the specific patient; a coracoid guide configured to conform to a surface of a coracoid process of the specific patient; and a glenoid guide configured to conform to a surface of a glenoid of the specific patient. 5. The set of patient-specific guides of claim 4 wherein the graft positioning guide defines one or more holes adapted for locating one or more fasteners to bind the corticoid graft to a prepared surface of the glenoid. 6. The set of patient-specific guides of claim 5 wherein coracoid guide comprises: an osteotomy cutting slot configured to receive a cutting element therethrough to guide removal of the coracoid process from a scapula of the specific patient; a medial cutting slot configured to receive the cutting element therethrough to guide preparation of a medial surface of the removed coracoid process; and one or more guide holes adapted to guide the creation of one or more holes in the coracoid process. 7. The set of patient-specific guides of claim 6 wherein the glenoid guide comprises: a decortication cutting slot configured to receive a cutting element therethrough to guide decortication of a margin of a glenoid of the specific patient; and one or more guide holes adapted to guide the creation of one or more holes in the decorticated margin of the glenoid of the specific patient. 8. The set of patient-specific guides of claim 7 wherein the one or more holes defined in the graft positioning guide are configured to align with the one or more holes in the coracoid process and the one or more holes in the decorticated margin of the glenoid. 9. The set of patient-specific guides of claim 8 wherein the surface of the removed coracoid process prepared using the coracoid guide corresponds with the decorticated margin of the glenoid prepared using the glenoid guide when held in place by the graft positioning guide.
