Catheter-based delivery systems for delivery and retrieval of a leadless pacemaker include features to facilitate improved manipulation of the catheter and improved capture and docking functionality of leadless pacemakers. Such functionality includes mechanisms directed to deflecting and locking a deflectable catheter, maintaining tension on a retrieval feature, protection from anti-rotation, and improved docking cap and drive gear assemblies.

What is claimed is: 1. A system, comprising: an implantable medical device configured to at least one of sense activity of, or deliver stimulation to, a heart; and a retrieval system comprising: a catheter shaft; a snare extending through the catheter shaft, the snare configured to grasp the implanted medical device; and a handle coupled to a proximal end of the catheter shaft, the handle comprising: a first handle portion; and a second handle portion disposed at least partially within the first handle portion and coupled to the snare such that the snare may be retracted into the catheter shaft by displacing the second handle portion relative to the first handle portion; and a ratchet selectively coupling the first handle portion and the second handle portion, the ratchet configured to allow proximal movement of the second handle portion relative to the first handle portion to retract the snare toward the catheter shaft, the ratchet configured to resist distal movement of the second handle portion relative to the first handle portion. 2. The system of claim 1 , wherein the first handle portion comprises a pawl and the second handle portion comprises a rack, the ratchet including the pawl and the rack. 3. The system of claim 1 , wherein the first handle portion comprises a release that, when actuated, reduces resistance to distal movement of the second handle portion relative to the first handle portion thereby allowing the snare to release the implantable medical device. 4. The system of claim 3 , wherein the release comprises at least one button disposed on an exterior surface of the first handle portion, the release configured to release the snare from the implantable medical device. 5. The system of claim 1 , wherein the first handle portion is movable between a first position in which the ratchet does not couple the first handle portion and the second handle portion and a second position in which the ratchet couples the first handle portion and the second handle portion. 6. The system of claim 1 , wherein the second handle portion is rotatable within the first handle portion when the second handle portion is in the second position. 7. The system of claim 1 , wherein the ratchet is configured to resist the distal movement of the second handle portion relative to the first handle portion to maintain tension on the snare. 8. The system of claim 1 , wherein the catheter shaft includes a distal end configured to engage the implantable medical device, the snare configured to dock the distal end with the implantable medical device when the snare grasps the implanted medical device. 9. The system of claim 1 , wherein the catheter shaft is steerable, the handle further comprising a mechanism configured to actuate deflection of a tip of the catheter shaft in connection with steering the tip of the catheter shaft to and within a chamber of the heart.
