Abstract

Official abstract text for this publication.

A medical device, comprising a fastening assembly, wherein the fastening assembly includes an adapter defining an opening therethrough, a fastener releasably coupled to the adapter, wherein the fastener includes at least two jaws pivotable relative to one another, the fastener having a first configuration wherein the jaws are open and a second configuration wherein the jaws are closed, and at least one cable coupled to the fastener and the adapter, the at least one cable being configured to apply tension to the fastener to transition between the first configuration and the second configuration while the fastener is coupled to the adapter, and to control release of the fastener from the adapter.

What is claimed is: 1. A medical device, comprising: a fastening assembly, wherein the fastening assembly includes: an adapter defining an opening therethrough; a fastener releasably coupled to the adapter, wherein the fastener includes at least two jaws pivotable relative to one another, the fastener having a first configuration wherein the at least two jaws are open and a second configuration wherein the at least two jaws are closed; a first cable and a second cable coupled to the fastener and the adapter, wherein the first cable and the second cable are configured to apply tension to the fastener to transition between the first configuration and the second configuration while the fastener is coupled to the adapter; and a third cable coupled to the adapter, wherein the third cable is configured to move the adapter and to control release of the fastener from the adapter. 2. The medical device of claim 1 , wherein the adapter has a tubular shape with a longitudinal opening configured to receive a distal end of a shaft, wherein a proximal portion of the adapter includes a plurality of circumferential segments, wherein the plurality of circumferential segments are biased radially inwards. 3. The medical device of claim 1 , wherein the at least two jaws comprise a first jaw coupled to a second jaw at each of a first pivot and a second pivot. 4. The medical device of claim 3 , wherein the first cable is coupled to the first pivot and the second cable is coupled to the second pivot, wherein tensioning the first cable and the second cable pivots the first jaw relative to the second jaw. 5. The medical device of claim 1 , wherein the fastening assembly further includes: a connector, wherein the connector defines a lumen, and wherein a proximal end of the adapter is movably received within the connector; and a spring disposed within the lumen of the connector, such that the proximal end of the adapter compresses the spring as the third cable moves the adapter proximally. 6. The medical device of claim 1 , wherein each of the first cable and the second cable includes a distal loop. 7. The medical device of claim 1 , further comprising a shaft and a handle operably coupled to a proximal end of the shaft, wherein the adapter is coupled to a distal portion of the shaft and the first cable and the second cable extend through the shaft from the adapter to the handle. 8. The medical device of claim 7 , wherein the third cable is configured to move the adapter proximally, such that a portion of the first cable and a portion of the second cable are sheared between the shaft and the adapter. 9. A medical device, comprising: a fastening assembly, wherein the fastening assembly includes: an adapter; a fastener releasably coupled to the adapter, wherein the fastener includes a first jaw pivotable relative to a second jaw, the first jaw and the second jaw being open in a first configuration of the fastener and closed in a second configuration of the fastener; a first cable and a second cable coupled to the fastener, wherein tensioning the first cable transitions the fastener between the first configuration and the second configuration; and a third cable coupled to the adapter, wherein tensioning the third cable releases the fastener from the adapter, wherein tensioning the third cable translates the fastener distally relative to the adapter. 10. The medical device of claim 9 , further comprising a shaft, a connector, and a handle operably coupled to a proximal end of the shaft, wherein the adapter is coupled to a distal portion of the connector, the connector is coupled to a distal portion of the shaft, and the first cable, the second cable, and the third cable extend proximally through the connector from the adapter to the handle. 11. The medical device of claim 10 , wherein the medical device further comprises a spring within a lumen of the connector adjacent to the adapter. 12. The medical device of claim 11 , wherein tensioning the third cable moves the adapter proximally, against a resistance of the spring. 13. The medical device of claim 12 , wherein the proximal movement of the adapter shears the first cable and the second cable from the fastener, thereby releasing the fastener from the adapter. 14. The medical device of claim 13 , wherein the proximal movement of the adapter shears the first cable and the second cable between the shaft and the adapter. 15. A method of fastening tissue, comprising: inserting a medical device into a natural orifice of a subject, wherein the medical device includes a fastening assembly coupled to a distal portion of a shaft of the medical device, the fastening assembly including a fastener, an adapter, and a first cable and a second cable coupled to the fastener, and a third cable coupled to the adapter; positioning the fastening assembly adjacent to a target site; applying a slack to the first cable and the second cable to close the fastener onto tissue at the target site while the fastener is coupled to the adapter; and applying a tension to the third cable to release the fastener from the adapter, wherein the applying the tension to the third cable causes the adapter to move proximally relative to the shaft. 16. The method of claim 15 , further comprising, after applying the slack to the first cable and the second cable before applying the tension to the third cable, applying a tension to the first cable and the second cable in order to open the fastener. 17. The method of claim 15 , wherein the proximal movement of the adapter relative to the shaft shears the first cable and the second cable between the adapter and the shaft, thereby releasing the fastener from the adapter. 18. The method of claim 15 , further comprising moving the adapter proximally, wherein the proximal movement of the adapter compresses a spring of the fastening assembly.
