An intravascular catheter for peri-vascular and/or peri-urethral tissue ablation includes multiple needles advanced through supported guide tubes which expand with open ends around a central axis to engage the interior surface of the wall of the renal artery or other vessel of a human body allowing the injection an ablative fluid for ablating tissue, and/or nerve fibers in the outer layer or deep to the outer layer of the vessel, or in prostatic tissue. The system also includes means to limit and/or adjust the depth of penetration of the ablative fluid into and beyond the tissue of the vessel wall. The preferred embodiment of the catheter includes structures which provide radial and lateral support to the guide tubes so that the guide tubes open uniformly and maintain their position against the interior surface of the vessel wall as the sharpened injection needles are advanced to penetrate into the vessel wall.

What is claimed is: 1. A method comprising: advancing a distal portion of a catheter having at least three needle guide elements and at least three needles to an artery within a human body; advancing the at least three needle guide elements to expand outwardly against a wall of the artery, wherein the at least three needle guide elements center the distal portion of the catheter relative to the artery in preparation for deployment of the least three needles; advancing the at least three needles through the at least three needle guide elements into tissue after the at least three needle guide elements center the distal portion of the catheter, wherein the at least three needles are advanced at least 2 mm beyond the wall of the artery; and injecting fluid into the tissue through the at least three needles. 2. The method of claim 1 , wherein injecting fluid comprises treating hypertension. 3. The method of claim 1 , wherein the fluid comprises ethanol, distilled water, hypertonic saline, phenol, glycerol, lidocaine, bupivacaine, tetracaine, benzocaine, guanethidine, botulinum toxin, glycoside, or neurotoxic fluid. 4. The method of claim 1 , wherein injecting fluid comprises injecting less than 1 ml of fluid. 5. The method of claim 1 , wherein the artery is the renal artery and injecting fluid comprises ablating nerves in the adventitial and peri-adventitial layers of the renal artery. 6. The method of claim 1 , wherein the at least three needles are advanced through the internal elastic lamina and media of the artery. 7. A method comprising: advancing a distal portion of a catheter having at least three needle guide elements and at least three needles to a vein within a human body; advancing the at least three needle guide elements to expand outwardly against a wall of the vein in a human body, wherein the at least three needle guide elements center the distal portion of the catheter relative to the vein in preparation for deployment of the least three needles; advancing the at least three needles through the at least three needle guide elements into tissue after the at least three needle guide elements center the distal portion of the catheter, wherein the at least three needles are advanced at least 2 mm beyond the wall of the vein; and injecting fluid into the tissue through the at least three needles. 8. The method of claim 7 , wherein the vein is the pulmonary vein and injecting fluid comprises treating atrial fibrillation. 9. The method of claim 7 , wherein injecting fluid comprises treating cardiac arrhythmias. 10. The method of claim 7 , wherein the fluid comprises ethanol, distilled water, hypertonic saline, phenol, glycerol, lidocaine, bupivacaine, tetracaine, benzocaine, guanethidine, botulinum toxin, glycoside, or neurotoxic fluid. 11. The method of claim 7 , wherein the fluid comprises anti-arrhythmic drugs that have neurotoxicity, botulinum toxin, digoxin or other cardiac glycosides. 12. A method comprising: advancing a distal portion of a catheter having at least three needle guide elements and at least three needles to a urethra within a human body; advancing the at least three needle guide elements to expand outwardly against a wall of the urethra, wherein the at least three needle guide elements center the distal portion of the catheter relative to the urethra in preparation for deployment of the least three needles; advancing the at least three needles through the at least three needle guide elements into prostatic tissue after the at least three needle guide elements center the distal portion of the catheter, wherein the at least three needles are advanced at least 2 mm beyond the wall of the urethra; and injecting fluid into the prostatic tissue through the at least three needles. 13. The method of claim 12 , wherein injecting fluid comprises treating benign prostatic hypertrophy. 14. The method of claim 12 , wherein injecting fluid comprises treating prostate cancer. 15. The method of claim 12 , wherein the fluid comprises ethanol, distilled water, hypertonic saline, phenol, glycerol, lidocaine, bupivacaine, tetracaine, benzocaine, guanethidine, botulinum toxin, glycoside, or neurotoxic fluid. 16. The method of claim 12 , wherein injecting fluid into prostatic tissue comprises local drug delivery into prostatic tissue. 17. The method of claim 12 , wherein injecting fluid into prostatic tissue comprises delivery of neurotoxic agents.
