A bone positioning guide may include a main body member, a shaft, and a bone engagement member. The shaft can be movably connected to the main body member and have the bone engagement member rotatably coupled to its distal end. The bone engagement member can have a surface configured to engage a bone. The main body member can also include a tip opposite the bone engagement member for engaging a second bone. In use, the bone engagement member may be positioned in contact with a medial side of a first metatarsal while the tip is positioned in contact with a lateral side of a different metatarsal, such as a second metatarsal. The shaft can then be moved to advance the bone engagement member toward the tip, causing realignment of the first metatarsal.

The invention claimed is: 1. A bone positioning system comprising: a bone positioning guide operable to move a metatarsal in at least one plane to correct an alignment of the metatarsal relative to an opposed cuneiform; a tissue removing instrument configured to prepare an end of the metatarsal and an end of the opposed cuneiform for fusion; and a fixation device configured to be applied to the metatarsal and the opposed cuneiform across a tarsometatarsal joint to fixate the end of the metatarsal relative to the end of the opposed cuneiform for fusion across the tarsometatarsal joint. 2. The bone positioning system of claim 1 , wherein the bone positioning guide is operable to move the metatarsal in multiple planes, including about an axis in a frontal plane. 3. The bone positioning system of claim 1 , wherein the metatarsal is a first metatarsal and the opposed cuneiform is a medial cuneiform. 4. The bone positioning system of claim 1 , wherein the fixation device comprises one or more of a compressing bone screw, a bone plate, and a bone staple. 5. The bone positioning system of claim 1 , wherein the fixation device comprises a bone plate. 6. The bone positioning system of claim 1 , wherein the bone positioning guide comprises: a bone engagement member; a tip; and a mechanism configured to move the bone engagement member and the tip relative to each other. 7. The bone positioning system of claim 6 , wherein the bone positioning guide comprises an actuator configured to actuate the mechanism. 8. The bone positioning system of claim 1 , wherein the tissue removing instrument comprises a saw. 9. The bone positioning system of claim 1 , wherein the tissue removing instrument comprises a bur. 10. The bone positioning system of claim 1 , wherein the tissue removing instrument comprises an osteotome. 11. The bone positioning system of claim 1 , further comprising a temporary fixation device configured to be applied to the metatarsal to temporarily fixate a moved position of the metatarsal prior to permanent fixation using the fixation device. 12. The bone positioning system of claim 11 , wherein the temporary fixation device comprises a fixation wire. 13. A bone positioning system comprising: a bone positioning guide operable to move a first metatarsal in multiple planes relative to a medial cuneiform, including about an axis in a frontal plane, to correct a bunion; a tissue removing instrument configured to prepare an end of the first metatarsal and an end of the medial cuneiform for fusion; and a fixation device configured to be applied to the first metatarsal and the medial cuneiform across a tarsometatarsal joint to fixate the end of the first metatarsal relative to the end of the medial cuneiform for fusion across the tarsometatarsal joint. 14. The bone positioning system of claim 13 , wherein the fixation device comprises one or more of a compressing bone screw, a bone plate, and a bone staple. 15. The bone positioning system of claim 13 , wherein the fixation device comprises a bone plate. 16. The bone positioning system of claim 13 , wherein the bone positioning guide comprises: a bone engagement member; a tip; and a mechanism configured to move the bone engagement member and the tip relative to each other. 17. The bone positioning system of claim 16 , wherein the bone positioning guide comprises an actuator configured to actuate the mechanism. 18. The bone positioning system of claim 13 , wherein the tissue removing instrument comprises one or more of a saw, a bur, and an osteotome. 19. The bone positioning system of claim 13 , further comprising a temporary fixation device configured to be applied to the first metatarsal to temporarily fixate a moved position of the first metatarsal prior to permanent fixation using the fixation device. 20. The bone positioning system of claim 19 , wherein the temporary fixation device comprises a fixation wire.
