Abstract

Official abstract text for this publication.

A delivery apparatus for a prosthetic heart valve includes a shaft, an inner balloon, and an outer balloon. The shaft has a lumen extending from a proximal end portion to a distal end portion and a plurality of openings formed in the distal end portion. The shaft is configured such that a fluid can flow through the lumen and the openings. The inner balloon has end portions and a center portion disposed between the end portions. The inner balloon is mounted on the distal end portion of the shaft and is in fluid communication with the openings of the shaft. When the inner balloon is inflated with the fluid, the end portions expand farther radially outwardly than the center portion. The outer balloon is mounted to the shaft and disposed over the inner balloon. The outer balloon is configured to fully expand after the inner balloon at least partially expands.

First claim

We claim: 1. A method of positioning a prosthetic heart valve, comprising: inserting a distal end portion of a delivery apparatus into a patient's vasculature, wherein the distal end portion comprises a prosthetic heart valve mounted thereon; advancing the distal end portion of the delivery apparatus and the prosthetic heart valve to an implantation location; partially inflating an inner balloon of the delivery apparatus disposed at the distal end portion of the delivery apparatus with a fluid, wherein partially inflating the inner balloon results in end portions of the inner balloon expanding radially outwardly farther than at a center portion of the inner balloon, and wherein partially inflating the inner balloon results in partial expansion of the prosthetic heart valve; after partially inflating the inner balloon, fully inflating an outer balloon of the delivery apparatus disposed at the distal end portion of the delivery apparatus with the fluid, wherein the fluid flows from within the inner balloon, through openings in the inner balloon, and into the outer balloon, wherein the outer balloon is sealed so as to prevent fluid from flowing out of the outer balloon into the patient's vasculature, and wherein fully inflating the outer balloon results in full expansion of the prosthetic heart valve; and prior to fully inflating the outer balloon, inflating the inner balloon until the inner balloon burst. 2. The method of claim 1 , wherein the outer balloon is configured to fully expand after the inner balloon bursts. 3. The method of claim 1 , wherein the inner balloon is configured to burst at a predetermined pressure. 4. The method of claim 3 , wherein the predetermined pressure is 1-5 bar. 5. The method of claim 1 , wherein the inner balloon has a dog bone shape when partially inflated. 6. The method of claim 1 , wherein the outer balloon has a cylindrical shape when fully inflated. 7. The method of claim 1 , wherein when the outer balloon is fully inflated, the outer balloon is radially spaced from the inner balloon. 8. The method of claim 1 , wherein the distal end portion of the delivery apparatus and the prosthetic heart valve are advanced to the implantation location using a transfemoral delivery procedure. 9. A method of positioning a prosthetic heart valve, comprising: inserting a distal end portion of a delivery apparatus into a patient's vasculature, wherein the distal end portion comprises a prosthetic heart valve mounted thereon; advancing the distal end portion of the delivery apparatus and the prosthetic heart valve to an implantation location; inflating an inner balloon to a first pressure with a fluid, wherein inflating the inner balloon to the first pressure results in partial expansion of the prosthetic heart valve; partially inflating an outer balloon of the delivery apparatus disposed at the distal end portion of the delivery apparatus with the fluid, wherein the outer balloon is sealed so as to prevent fluid from flowing out of the outer balloon into the patient's vasculature, and wherein partially inflating the outer balloon comprises inflating the outer balloon with the fluid from the inner balloon flowing into the outer balloon from pores of the inner balloon; inflating the inner balloon from the first pressure to a second pressure, wherein the second pressure is greater than the first pressure, and wherein the inner balloon is configured to rupture at the second pressure; and after rupturing the inner balloon, fully inflating the outer balloon, wherein fully inflating the outer balloon results in full expansion of the prosthetic heart valve. 10. The method of claim 9 , wherein when the inner balloon is inflated to the first pressure, end portions of the inner balloon are bulbous and taper radially inwardly to a center portion of the inner balloon. 11. The method of claim 9 , wherein when the inner balloon is inflated to the first pressure, a center portion of the inner balloon is cylindrical. 12. The method of claim 9 , wherein the second pressure is 1-5 bar. 13. The method of claim 9 , wherein advancing the distal end portion of the delivery apparatus and the prosthetic heart valve to the implantation location comprises using a transfemoral delivery procedure. 14. A method of positioning a prosthetic heart valve, comprising: inserting a shaft of a delivery apparatus into a patient's vasculature using a transfemoral route, wherein a prosthetic heart valve is mounted on the shaft at a first location; advancing the shaft and the prosthetic heart valve to a first position within the patient's vasculature; moving the prosthetic heart valve relative to the shaft from the first location to a second location, wherein the second location comprises an inner balloon and an outer balloon radially disposed between the prosthetic heart valve and the shaft; advancing the shaft and the prosthetic heart valve from the first position to second position; partially inflating the inner balloon with a fluid, wherein partially inflating the inner balloon results in end portions of the inner balloon expanding radially outwardly farther than at a center portion of the inner balloon, and wherein partially inflating the inner balloon results in partial expansion of the prosthetic heart valve; and after partially inflating the inner balloon, fully inflating the outer balloon with the fluid, wherein the fluid flows from within the inner balloon, through openings in the inner balloon, and into the outer balloon, wherein the outer balloon is sealed so as to prevent fluid from flowing out of the outer balloon into the patient's vasculature, and wherein fully inflating the outer balloon results in full expansion of the prosthetic heart valve; and prior to fully inflating the outer balloon, inflating the inner balloon until the inner balloon burst. 15. The method of claim 14 , wherein the first location of the shaft is proximal to the second location of the shaft. 16. The method of claim 15 , wherein the outer balloon is configured to fully expand after the inner balloon bursts. 17. The method of claim 16 , wherein the inner balloon is configured to burst at a predetermined pressure of 1-5 bar.
