The invention provides improved methods for cell therapy. In particular, the invention provides therapeutic compositions of enhanced hematopoietic stem and progenitor cells having improved engraftment and homing properties, and methods of making the therapeutic compositions. The invention further provides methods of improving the efficacy of hematopoietic stem and progenitor cell transplantation including transplanting the therapeutic composition to subjects in need of hematopoietic system reconstitution.

First claim

The invention claimed is: 1. A human hematopoietic stem or progenitor cell (HSPC) that has been contacted ex vivo with agents that increase CXCR4 gene expression in the cell by at least about 30 fold in the contacted HSPC compared to a HSPC that has not been contacted ex vivo with the agents that increase CXCR4 gene expression, wherein the agents comprises (i) one or more prostaglandin pathway agonists; and (ii) one or more glucocorticoids, and wherein the one or more prostaglandin pathway agonist comprises a compound that selectively binds the PGE2 EP2 or PGE2 EP4 receptor. 2. The human HSPC of claim 1 , wherein the CXCR4 expression four hours after beginning contact with the agents is at least 30-fold greater than a population of HSPCs not contacted ex vivo with the agents that increase CXCR4 expression. 3. The human HSPC of claim 1 , wherein the one or more prostaglandin pathway agonist is selected from the group consisting of PGE2, dmPGE2, 15(S)-15-methyl PGE2, 20-ethyl PGE2, and 8-iso-16-cyclohexyl-tetranor PGE2. 4. The human HSPC of claim 2 , wherein the one or more prostaglandin pathway agonist comprises 16,16-dimethyl PGE2. 5. The human HSPC of claim 2 , wherein the glucocorticoid is selected from the group consisting of medrysone, alclometasone, alclometasone dipropionate, amcinonide, beclometasone, beclomethasone dipropionate, betamethasone, betamethasone benzoate, betamethasone valerate, budesonide, ciclesonide, clobetasol, clobetasol butyrate, clobetasol propionate, clobetasone, clocortolone, cloprednol, Cortisol, cortisone, cortivazol, deflazacort, desonide, desoximetasone, desoxycortone, dexamethasone, diflorasone, diflorasone diacetate, diflucortolone, diflucortolone valerate, difluorocortolone, difluprednate, fluclorolone, fluclorolone acetonide, fludroxycortide, flumethasone, flumethasone pivalate, flunisolide, flunisolide hemihydrate, fluocinolone, fluocinolone acetonide, fluocinonide, fluocortin, fluocoritin butyl, fluocortolone, fluorocortisone, fluorometholone, fluperolone, fluprednidene, fluprednidene acetate, fluprednisolone, fluticasone, fluticasone propionate, formocortal, halcinonide, halometasone, hydrocortisone, hydrocortisone acetate, hydrocortisone aceponate, hydrocortisone buteprate, hydrocortisone butyrate, loteprednol, meprednisone, 6a-methylprednisolone, methylprednisolone, methylprednisolone acetate, methylprednisolone aceponate, mometasone, mometasone furoate, mometasone furoate monohydrate, paramethasone, prednicarbate, prednisolone, prednisone, prednylidene, rimexolone, tixocortol, triamcinolone, triamcinolone acetonide and ulobetasol. 6. The human HSPC of claim 1 , wherein the agents comprise 16,16-dimethyl PGE2 and dexamethasone. 7. An ex vivo modified cell-based composition comprising hematopoietic stem or progenitor cells (HSPCs) wherein at least 50% of the cells express CXCR4 and CD34, wherein the expression of CXCR4 in the HSPCs is at least 30-fold greater than a population of untreated HSPC, wherein the HSPCs have been contacted with agents, and wherein the agents comprise (i) one or more prostaglandin pathway agonists; and (ii) one or more glucocorticoids. 8. The ex vivo modified cell-based composition of claim 7 , wherein expression of CXCR4 in the HSPCs contacted with the agents is at least 30-fold greater than HSPCs not contacted with the agents. 9. The ex vivo modified cell-based composition of claim 8 , wherein the expression four hours after beginning contact with the agents is at least 30-fold greater than a population of HSPCs not contacted with the agents. 10. The cell-based composition of claim 7 , wherein the percentage of HSPCs expressing CXCR4 is increased by at least 12-fold compared to HSPCs not contacted with the agents. 11. The ex vivo modified cell-based composition of claim 7 , wherein the one or more prostaglandin pathway agonists comprises PGE2 or a PGE2 analogue or derivative thereof. 12. The ex vivo modified cell-based composition of claim 7 , wherein the one or more glucocorticoids is selected from the group consisting of medrysone, alclometasone, alclometasone dipropionate, amcinonide, beclometasone, beclomethasone dipropionate, betamethasone, betamethasone benzoate, betamethasone valerate, budesonide, ciclesonide, clobetasol, clobetasol butyrate, clobetasol propionate, clobetasone, clocortolone, cloprednol, cortisol, cortisone, cortivazol, deflazacort, desonide, desoximetasone, desoxycortone, desoxymethasone, dexamethasone, diflorasone, diflorasone diacetate, diflucortolone, diflucortolone valerate, difluorocortolone, difluprednate, fluclorolone, fluclorolone acetonide, fludroxycortide, flumetasone, flumethasone, flumethasone pivalate, flunisolide, flunisolide hemihydrate, fluocinolone, fluocinolone acetonide, fluocinonide, fluocortin, fluocoritin butyl, fluocortolone, fluorocortisone, fluorometholone, fluperolone, fluprednidene, fluprednidene acetate, fluprednisolone, fluticasone, fluticasone propionate, formocortal, halcinonide, halometasone, hydrocortisone, hydrocortisone acetate, hydrocortisone aceponate, hydrocortisone buteprate, hydrocortisone butyrate, loteprednol, meprednisone, 6amethylprednisolone, methylprednisolone, methylprednisolone acetate, methylprednisolone aceponate, mometasone, mometasone furoate, mometasone furoate monohydrate, paramethasone, prednicarbate, prednisolone, prednisone, prednylidene, rimexolone, tixocortol, triamcinolone, triamcinolone acetonide and ulobetasol. 13. The ex vivo modified cell-based composition of claim 7 , wherein the prostaglandin pathway agonist is 16,16-dimethyl PGE2 and the glucocorticoid is dexamethasone. 14. The ex vivo modified cell-based composition of claim 7 , wherein the HSPCs are obtained from bone marrow, umbilical cord blood, mobilized peripheral blood, Wharton's jelly, placenta, fetal blood, or induced pluripotent stem cells (iPSCs). 15. The ex vivo modified cell-based composition of claim 7 , wherein the HSPCs are contacted with the agents at a temperature of about 22° C. to about 39° C. 16. The ex vivo modified cell-based composition of claim 7 , wherein the HSPCs are not expanded ex vivo.
