An insertion handle for medical implants includes a handle with an elongate shaft extending therefrom and connection means for the implant disposed at the end of the shaft opposite the handle. The connection means includes a pivotable attachment for the implant that is controlled remotely from the handle. Both angle of the implant with respect to the handle and shaft as well as the attachment may be separately controlled and adjusted. Remote angular adjustment facilitates insertion of implants in to small surgical sites because the orientation of the implant may be repeatedly, remotely adjusted as the implant is inserted. Connectors may also be provided at the engagement surface between the handle and implant in order to provide communication with the implant or surgical site. The connectors also may serve as torque bearing members to avoid the need for separate torque bearing means such as keyways and the like.

The invention claimed is: 1. An instrument for inserting a medical implant, comprising: a handle; an outer shaft extending from the handle along a shaft axis; an inner shaft extending at least partially through the outer shaft; an actuator coupled to the handle and configured to cause movement of the inner shaft within the outer shaft; and an attachment screw for engagement with a medical implant, the attachment screw extending through an elongated window opening, and the attachment screw being pivotable about a pivot axis extending transverse to the shaft axis such that a longitudinal axis of the attachment screw is pivotable between a plurality of angles within the elongated window opening, wherein the attachment screw is coupled with the inner shaft to permit rotational drive of the attachment screw about its longitudinal axis while the attachment screw is positioned at any of the plurality of angles. 2. The instrument of claim 1 , further comprising a locking mechanism operable to selectively fix the angle of the attachment screw. 3. The instrument of claim 1 , wherein the attachment screw is coupled with the inner shaft via a universal joint. 4. The instrument of claim 1 , further comprising a pivot head received within a cage, wherein the attachment screw is received within the pivot head, and wherein the pivot head is rotatable about the pivot axis to pivot the attachment screw between the plurality of angles. 5. The instrument of claim 4 , further including an implant interface configured to engage the implant, wherein the attachment screw extends through the implant interface, and wherein the implant interface is integrally formed as a monolithic member with the pivot head. 6. The instrument of claim 4 , further comprising a locking mechanism operable to selectively fix the angle of the attachment screw. 7. The instrument of claim 6 , wherein the cage defines a wall that is deformable such that an inner surface of the wall compressively engages the pivot head to fix the angle of the attachment screw. 8. The instrument of claim 6 , wherein the cage is adapted to constrict around the pivot head to fix the angle of the attachment screw. 9. The instrument of claim 6 , wherein the locking mechanism includes a pivot lock that advances into engagement with the pivot head to fix the angle of the attachment screw. 10. The instrument of claim 9 , wherein the handle includes a pivot lock actuator to control the advancement of the pivot lock. 11. The instrument of claim 9 , wherein the pivot lock includes an engagement surface for engaging a corresponding surface of the pivot head, and wherein the engagement surface is shaped to conform to a shape of the corresponding surface of the pivot head. 12. The instrument of claim 11 , wherein the engagement surface of the pivot lock includes a plurality of first facets arranged to engage a plurality of second facets on the corresponding surface of the pivot head. 13. The instrument of claim 11 , wherein the engagement surface of the pivot lock includes a plurality of first teeth arranged to mesh with a plurality of second teeth on the corresponding surface of the pivot head. 14. The instrument of claim 11 , wherein the engagement surface of the pivot lock includes a plurality of recesses. 15. The instrument of claim 11 , wherein the corresponding surface of the pivot head includes a plurality of recesses. 16. The instrument of claim 1 , further including an implant interface configured to engage the implant, wherein the attachment screw extends through the implant interface, and wherein the implant interface is pivotable along with the attachment screw about the pivot axis. 17. The instrument of claim 16 , wherein the implant interface includes a connector extending therefrom and configured to engage the implant to prevent relative rotation between the implant and the implant interface. 18. The instrument of claim 16 , wherein the implant interface includes a passage for receiving a tube for providing fluid communication with a port on the implant. 19. The instrument of claim 1 , further including a tube for providing fluid communication with a port on the implant, the tube extending along and external to the outer shaft. 20. The instrument of claim 19 , further including a shaft clip for coupling the tube to the outer shaft.
