A hemodialysis vascular access device includes a proximal end sized and shaped to sealably couple to a hemodialysis catheter, a movable structure coupled to the proximal end, a fixation structure coupled to the movable region and sized and shaped for fixation on a patient, an elongated sleeve coupled to the fixation structure and sized and shaped for insertion into a patient's vasculature, and a valve at a distal end of the internal lumen. When a hemodialysis catheter is inserted into the device and coupled to the proximal end, distal movement of the proximal end relative to the fixation structure biases a distal end of the hemodialysis catheter from a position inside the elongated sleeve through a valve out of the device and into the patient's blood.

What is claimed is: 1. A hemodialysis device comprising: a dialysis catheter having a proximal end and a distal end; and a vascular access device comprising: a compressible proximal end having an extended configuration and a compressed configuration, the proximal end defining a proximal opening configured to receive the distal end of the dialysis catheter; a distal end comprising a sleeve, the sleeve defining a distal opening; a lumen extending through the sleeve and the compressible proximal end; a movable cap disposed at the proximal opening, the movable cap configured to couple with the proximal end of the dialysis catheter; and a valve disposed at the distal opening; wherein, when the proximal end of the dialysis catheter is coupled to the movable cap, moving the compressible proximal end from the extended configuration to the compressed configuration moves the distal end of the dialysis catheter through the lumen and out of the valve. 2. The hemodialysis device of claim 1 , wherein the valve includes a plurality of leaflets. 3. The hemodialysis device of claim 1 , wherein the valve includes a dome tip leaflet valve. 4. The hemodialysis device of claim 1 , further comprising a proximal valve that allows air to pass out of the compressible proximal end when the compressible proximal end is compressed. 5. The hemodialysis device of claim 1 , wherein at least a portion of the vascular access device has a hydrophilic coated surface. 6. The hemodialysis device of claim 1 , wherein the compressible proximal end further comprises an antimicrobial membrane. 7. The hemodialysis device of claim 1 , wherein the vascular access device further comprises a port for delivery of an antibiotic, anti-platelet antimicrobial, thrombolytic or anti-coagulant solution. 8. The hemodialysis device of claim 1 , wherein the proximal end of the dialysis catheter comprises a fixation point couplable with the movable cap. 9. The hemodialysis device of claim 8 , wherein the dialysis catheter has a captured length extending from a distal tip of the dialysis catheter to the fixation point; wherein the lumen of the vascular access device has a first length when the compressible proximal end is in the extended configuration and a second length smaller than the first length when the compressible proximal end is in the compressed configuration; and wherein the captured length of the dialysis catheter is smaller than the first length and larger than the second length. 10. The hemodialysis device of claim 1 , wherein a distal tip of the dialysis catheter can be selectively extended out of the distal opening or retracted into the lumen by moving the compressible proximal end between the extended configuration and the compressed configuration. 11. The hemodialysis device of claim 1 , wherein the compressible proximal end includes an air permeable membrane. 12. The hemodialysis device of claim 1 , wherein the compressible proximal end includes a balloon configured to inflate and deflate as the compressible proximal end moves between the extended configuration and the compressed configuration. 13. The hemodialysis device of claim 1 , wherein the valve is configured to prevent blood flow into the lumen. 14. The hemodialysis device of claim 1 , further comprising a fixation structure disposed between the sleeve and the compressible proximal end, the fixation structure configured to fix the vascular access device at an incision site in a patient's body. 15. A hemodialysis vascular access device comprising: a compressible proximal end sized and shaped to sealably couple to a hemodialysis catheter; a movable structure coupled to the compressible proximal end and sized and shaped to receive a portion of the hemodialysis catheter; a fixation structure coupled to the compressible proximal end, the fixation structure sized and shaped for fixation on a patient, the fixation structure sized and shaped to receive a portion of the hemodialysis catheter; a distal end comprising an elongated sleeve coupled to the fixation structure, the elongated sleeve sized and shaped for insertion into a patient's vasculature, the elongated sleeve having an internal lumen sized and shaped to receive a portion of the hemodialysis catheter; and a valve at a distal end of the internal lumen; wherein the vascular access device is configured such that, when the hemodialysis catheter is inserted into the vascular access device and coupled to the compressible proximal end, distal movement of the movable structure relative to the fixation structure forces a distal end of the hemodialysis catheter through the valve, out of the internal lumen, and into a patient's blood. 16. The hemodialysis vascular access device of claim 15 , wherein the movable structure comprises an air permeable membrane. 17. The hemodialysis vascular access device of claim 16 , further comprising a structural support inside the air permeable membrane. 18. The hemodialysis vascular access device of claim 15 , wherein the compressible proximal end further comprises at least one sealing valve for sealably coupling with the hemodialysis catheter.
