In certain embodiments, a vial adaptor for removing liquid contents from a vial comprises a piercing member and a bag. The bag can be contained within the piercing member such that the bag is introduced to the vial when the vial adaptor is coupled with the vial. In some embodiments, the bag expands within the vial as liquid is removed from the vial via the adaptor, thereby regulating pressure within the vial. In other embodiments, a vial comprises a bag for regulating pressure within the vial as liquid is removed therefrom. In some embodiments, a vial adaptor is coupled with the vial in order to remove the liquid. In some embodiments, as the liquid is removed from the vial via the adaptor, the bag expands within the vial, and in other embodiments, the bag contracts within the vial.

The invention claimed is: 1. An apparatus configured to aid in accessing contents of a vial, the apparatus comprising: a housing portion comprising: a medical connector interface configured to couple the apparatus with a medical device to place an interior of the medical device in fluid communication with an interior of the vial; and a substantially hollow sheath surrounding a chamber; an insertion member movable independent of the housing portion and configured to enter the chamber; a locking mechanism configured to secure the housing portion to the insertion member, wherein the locking mechanism comprises a ridge connected to the housing portion so that the ridge is formed integrally with the sheath, wherein the ridge is moved into an open position as the insertion member is entering the chamber and is returned to a closed position by a springing action thereafter, wherein in the closed position, the springing action forces the ridge against a portion of the insertion member to stabilize the insertion member inside the chamber; a regulator channel in the insertion member, wherein the regulator channel is configured to permit ambient air to flow into the regulator channel in response to medicinal fluid being withdrawn from the vial, thereby equalizing pressure within the vial; a piercing member configured to pierce a septum of the vial; a first filter, configured to absorb a first group of contaminants in a liquid state or a vapor state; and a second filter, distinct from the first filter, configured to absorb a second group of contaminants at least partially distinct from the first group of contaminants. 2. The apparatus of claim 1 , wherein the ridge is biased toward the closed position to facilitate holding the insertion member in place. 3. The apparatus of claim 1 , further comprising a sealing member configured to form a substantially airtight seal between the sheath and the insertion member. 4. The apparatus of claim 3 , wherein the sealing member comprises an O-ring. 5. The apparatus of claim 3 , wherein the sheath is substantially cylindrical. 6. The apparatus of claim 1 , further comprising a plurality of ridges comprising the ridge. 7. The apparatus of claim 6 , wherein at least one of the plurality of ridges is located on a side of the sheath. 8. The apparatus of claim 1 , wherein the chamber narrows toward a distal end of the sheath to facilitate restricting movement of the insertion member relative to the sheath with the insertion member advanced toward the distal end of the sheath. 9. The apparatus of claim 1 , wherein the second group of contaminants comprises one or more contaminants comprising at least one of dust, bacteria, or viruses. 10. An apparatus configured to aid in accessing contents of a vial, the apparatus comprising: a housing portion comprising: a medical connector interface configured to couple the apparatus with a medical device to place an interior of the medical device in fluid communication with an interior of the vial; and a sheath forming a chamber; an insertion member movable independent of the housing portion and configured to enter the chamber; a locking mechanism configured to secure the housing portion to the insertion member, wherein the locking mechanism comprises a ridge connected to the housing portion, wherein the ridge is configured to rest against the insertion member to stabilize the insertion member inside the chamber; a regulator channel, wherein the regulator channel is configured to permit ambient air to flow into the regulator channel in response to medicinal fluid being withdrawn from the vial, thereby equalizing pressure within the vial; and a piercing member configured to pierce a septum of the vial. 11. The apparatus of claim 10 , wherein the ridge is biased toward a closed position, wherein in the closed position, the ridge is configured to stabilize the insertion member inside the chamber. 12. The apparatus of claim 11 , wherein the ridge is biased toward the closed position by springing action to facilitate holding the insertion member in place. 13. The apparatus of claim 12 , wherein the ridge is moved into an open position as the insertion member is entering the chamber and is returned to the closed position by the springing action thereafter. 14. The apparatus of claim 11 , wherein the chamber narrows toward a distal end of the sheath to facilitate restricting movement of the insertion member relative to the sheath with the insertion member advanced toward the distal end of the sheath. 15. The apparatus of claim 10 , wherein the ridge is formed integrally with the sheath. 16. The apparatus of claim 10 , further comprising a sealing member configured to form a substantially airtight seal between the sheath and the insertion member. 17. The apparatus of claim 10 , further comprising a plurality of ridges comprising the ridge. 18. The apparatus of claim 17 , wherein at least one of the plurality of ridges is located on a surface of the sheath. 19. An apparatus configured to aid in accessing contents of a vial, the apparatus comprising: a housing portion comprising: a medical connector interface configured to couple the apparatus with a medical device to place an interior of the medical device in fluid communication with an interior of the vial; and a sheath forming a chamber; an insertion member movable independent of the housing portion and configured to enter the chamber; and a locking mechanism configured to secure the housing portion to the insertion member, wherein the locking mechanism comprises a ridge connected to the housing portion, wherein the ridge is configured to rest against the insertion member to stabilize the insertion member inside the chamber at the inserted position.
