This invention is directed to substituted acylanilide compounds and uses thereof in treating muscular dystrophies such as Duchenne muscular dystrophy and Becker muscular dystrophy and in improving or preserving lung function and cardiac function in a subject suffering from Duchenne muscular dystrophy.

What is claimed is: 1. A method of treating, reducing the severity of, reducing the incidence of, delaying the onset of, or reducing the pathogenesis of myotonic dystrophy, limb-girdle muscular dystrophy, facioscapulhumeral muscular dystrophy, congenital muscular dystrophy, oculopharyngeal muscular dystrophy, distal muscular dystrophy, or Emery-Dreifuss muscular dystrophy in a subject in need thereof, consisting essentially of the step of administering to said subject a selective androgen receptor modulator (SARM) compound represented by the structure of formula S-XXIII: or its optical isomer, pharmaceutically acceptable salt, or any combination thereof. 2. The method of claim 1 , wherein said administering comprises administering a pharmaceutical composition comprising said compound and/or its optical isomer, pharmaceutically acceptable salt, or any combination thereof; and a pharmaceutically acceptable carrier. 3. The method of claim 1 , wherein said method further increases the physical function of said subject. 4. The method of claim 1 , wherein said method further increases the quality of life of said subject. 5. The method of claim 1 , wherein said method further delays onset or improves symptoms of cardiomyopathy and respiratory function.
