Bone plate systems are provided for surgical implants and bone repair. The bone plate is multi-tiered for receiving bone anchors screws for securing a plurality of bones or bone fragments in a desired relationship. The plate members include throughbores for receiving a pivot base therein, with head ends of the bone anchors being secured in the pivot bases. The throughbores may permit and define a translation path for the pivot base and the bone anchor secured therein relative to the plate. Pivot members extending between the pivot base and the plate member facilitate pivoting and optional translation of the pivot base relative to the plate. With the bone anchor seated within the pivot base, an apparatus for inhibiting screw back out is employed.

We claim: 1. A method of securing a bone plate assembly to a bone, the method comprising: positioning a lower surface of a bone plate of the bone plate assembly in contact with the bone, the bone plate assembly including: the bone plate having an elongated throughbore extending through the bone plate from the lower surface to an upper surface opposite to the lower surface and along a through bore axis, the bone plate including floor portions adjacent the lower surface of the bone plate and extending laterally transverse to the bore axis inwardly into the elongated throughbore on either side thereof that define a narrow section of the elongated throughbore; and a pivot base in the throughbore, the pivot base including a generally rectangular rigid body having a top surface, a bottom surface, and opposite, outer side wall portions intermediate the top and bottom surfaces of the pivot base, the pivot base further includes a pivot member extending laterally outward away from one of the outer side wall portions and being supported on one of the bone plate floor portions, the pivot member pivotally connecting the pivot base to the bone plate and permitting the pivot base to pivot and translate relative to the bone plate in the elongated throughbore; engaging an instrument guide with the rigid body of the pivot base, wherein engaging the instrument guide with the rigid body of the pivot base includes advancing opposing projecting members of one of a shaft of the instrument guide and the pivot base at least partially into recesses formed in outer side wall portions of the other of the shaft of the instrument guide and the pivot base between the rigid body of the pivot base and walls of the elongated through bore of the bone plate; positioning the pivot base in a desired angular orientation relative to the bone plate via the instrument guide; advancing a shank of a bone screw through a throughbore of the instrument guide and an opening of the rigid body of the pivot base; driving the shank of the bone screw into engagement with the bone via a driver engaged to the bone screw and extending in the throughbore of the instrument guide; and seating a head of the bone screw in the opening of the rigid body of the pivot base without deforming the rigid body of the pivot base to secure the bone plate assembly to the bone. 2. The method of claim 1 , wherein engaging the pivot base comprises resiliently engaging the pivot base with a resilient portion of the instrument guide. 3. The method of claim 1 , including attaching a handle of the instrument guide to the shaft of the instrument guide, and forming a viewing opening down along the shaft between the shaft and the attached handle for viewing the engagement of the pivot base by the instrument guide. 4. The method of claim 1 , wherein the positioning of the pivot base includes translating the pivot base relative to the bone plate and positioning the pivot base at a desired position in the throughbore. 5. The method of claim 1 , wherein engaging the pivot base includes positioning a distal surface of the instrument guide against the top surface of the pivot base. 6. The method of claim 1 , wherein positioning the pivot base includes pivoting the pivot base relative to the bone plate via the instrument guide. 7. The method of claim 1 , wherein positioning the pivot base includes using the instrument guide to pivot the pivot base about a pivot axis defined by the pivot member connecting the pivot base to the bone plate. 8. The method of claim 1 , wherein the bone plate is elongated along a bone plate axis and positioning the pivot base includes pivoting the pivot base using the instrument guide about an axis extending orthogonal to the bone plate longitudinal axis. 9. The method of claim 1 , wherein advancing the shank of the bone screw through the throughbore of the instrument guide and the opening of the rigid body of the pivot base includes advancing a bone preparation tool through the throughbore of the instrument guide to prepare the bone prior to advancing the shank of the bone screw through the throughbore of the instrument guide. 10. The method of claim 1 , wherein engaging the instrument guide with the rigid body of the pivot base includes compressing the projecting members of the instrument guide against the rigid body of the pivot base. 11. The method of claim 1 , wherein engaging the instrument guide with the rigid body of the pivot base comprises shifting a proximal tubular member of the instrument guide along a distal, tubular member of the instrument guide and compressing a spring between the proximal and distal tubular members to clamp the projecting members of the instrument guide against the pivot base.
