Described are methods and materials for use in the treatment or prophylaxis of diseases, for example cognitive disorders, using diaminophenothiazines. In particular it relates to treatments having optimized pharmacokinetic properties, and dosage forms are intended to improve the relative cognitive or CNS benefits of the diaminophenothiazines, for instance compared to haematological effects.

First claim

The invention claimed is: 1. A method of treating a cognitive or CNS disorder in a patient, comprising orally administering to said patient a delayed-release formulation of a dosage unit containing a 3,7-diaminophenothiazine (DAPTZ) compound in stable crystalline reduced form as active ingredient, selected from compounds of the following formula and pharmaceutically acceptable salts, solvates, and hydrates thereof: wherein: each of R 1 and R 9 is independently selected from: —H; each of R 3NA and R 3NB is independently selected from: —H, C 2-4 alkenyl, and halogenated C 1-4 alkyl; each of R 7NA and R 7NB is independently selected from: —H, C 2-4 alkenyl, and halogenated C 1-4 alkyl; each of HX 1 and HX 2 is independently a protic acid, wherein X 1 is an element or compound forming the conjugate base of the protic acid HX 1 , and X 2 is an element or compound forming the conjugate base of the protic acid HX 2 ; and wherein if any of HX 1 and HX 2 are hydrohalic acids, then each are independently selected from HCl or HBr, wherein the delayed-release formulation releases less than 50% of the compound in 6 hours. 2. The method of claim 1 wherein the delayed-release formulation of the reduced DAPTZ compound contains between 100-1000 mg of the DAPTZ compound. 3. The method of claim 2 wherein the delayed-release formulation is given once per day to the patient. 4. The method of claim 1 wherein the delayed-release formulation releases less than 50% 8 hours. 5. The method of claim 1 wherein the dosage unit is gastroretained. 6. The method of claim 5 wherein the gastroretained dosage unit is held in the stomach for at least 30 minutes, and optionally at least 1, 2, 3, 4, 5, 6, 8, 12 hours or more. 7. The method of claim 5 wherein the gastroretained dosage unit comprises at least 50, 60, 70, 80, 90 or 100 mg DAPTZ compound. 8. The method of claim 5 wherein the gastroretained dosage unit comprises either (i) particles that have bioadhesive properties and a propensity to float on the stomach contents or (ii) a swelling system, in either case such as to achieve a size after ingestion of greater than 15 mm. 9. The method of claim 1 wherein the dosage unit is provided as a pharmaceutical composition comprising the DAPTZ compound and a pharmaceutically acceptable carrier, diluent, or excipient. 10. The method of claim 1 wherein the pharmaceutical composition is for a combination therapy and comprises in addition to the DAPTZ compound a further active ingredient selected from: a cholinesterase inhibitor; an NMDA receptor antagonist; a muscarinic receptor agonist; an inhibitor of conversion of amyloid precursor protein to beta-amyloid. 11. The method of claim 1 , wherein each of R 1 and R 9 is independently —H. 12. The method of claim 1 , wherein each of R 3NA and R 3NB and R 7NA and R 7NB is independently -Me, -Et, -nPr, -nBu, —CH 2 —CH═CH 2 , or —CF 3 . 13. The method of claim 1 , wherein the groups —N(R 3NA )(R 3NB ) and —N(R 7NA )(R 7NB ) are the same, and are selected from: —NMe 2 and -NEt 2 . 14. The method of claim 1 , wherein each of the groups —N(R 3NA )(R 3NB ) and —N(R 7NA )(R 7NB ) is: —NMe 2 . 15. The method of claim 1 , wherein each of HX 1 and HX 2 is independently a monoprotic acid. 16. The method of claim 1 , wherein each of HX 1 and HX 2 is independently a hydrohalide acid. 17. The method of claim 1 , wherein HX 1 and HX 2 are each HCl. 18. The method of claim 1 , wherein HX 1 and HX 2 are each HBr. 19. The method of claim 1 , wherein each of HX 1 and HX 2 is an organic acid. 20. The method of claim 19 wherein each of HX 1 and HX 2 is independently selected from H 2 CO 3 and CH 3 COOH. 21. The method of claim 1 , wherein: each of R 1 and R 9 is —H; each of R 3NA and R 3NB is -Me; each of R 7NA and R 7NB is -Me; each of HX 1 and HX 2 is independently a monoprotic acid. 22. The method of claim 1 wherein the cognitive or CNS disorder is a tauopathy and wherein the treatment of the tauopathy is such that the DAPTZ compound causes inhibition of the aggregation of the tau protein associated with said tauopathy and also dissolution of tau aggregates in the brain of the patient or subject. 23. The method of claim 22 wherein the cognitive or CNS disorder is selected from Alzheimer's disease, Pick's disease, Progressive supranuclear palsy (PSP), fronto-temporal dementia with parkinsonism linked to chromosome 17, disinhibition-dementia-parkinsonism-amyotrophy complex, pallido-ponto-nigral degeneration, Guam-ALS syndrome; pallido-nigro-luysian degeneration, cortico-basal degeneration; Down's syndrome; and postencephalitic parkinsonism; parkinsonism with dementia complex of Guam. 24. The method of claim 1 wherein the cognitive or CNS disorder is selected from: CJD, nvCJD, fatal familial insomnia, Gerstmann-Straussler-Scheinker syndrome or Kuru where the protein is prion protein; Huntington's disease where the protein is huntingtin; Spinocerebellar ataxia 1, 2, 3, or 7 where the protein is ataxin 1, 2, 3 or 7; Dentatorubropallidoluysian atrophy where the protein is atrophin; Spinal and bulbar muscular atrophy where the protein is androgen receptor; Familial encephalopathy with neuronal inclusion bodies where the protein is neuroserpin; Hereditary cerebral angiopathy (Icelandic) where the protein is cystatin C; and Amyotrophic lateral sclerosis where the protein is superoxide dismutase 1. 25. The method of claim 1 wherein the cognitive or CNS disorder is mild cognitive impairment. 26. The method of claim 1 wherein the cognitive or CNS disorder is a synucleinopathy, which is optionally Parkinson's Disease, dementia with Lewy bodies, multiple system atrophy, drug-induced parkinsonism, or pure autonomic failure (PAF). 27. The method of claim 1 wherein the patient also has a haematological disorder, the effects of which may otherwise be exacerbated by the DAPTZ compound. 28. The method of claim 27 wherein the haematological disorder is selected from a haemoglobinopathy, an anemia, a haematological malignancy, and a coagulopathy. 29. The method of claim 28 wherein: (i) the haemoglobinopathy is sickle-cell disease, thalassemia, or methaemoglobinemia; (ii) the anemia is a haemolytic anemia; (iii) the haematological malignancy is lymphoma, myeloma, plasmacytoma or leukemia; and (iv) the coagulopathy is hemophilia.
