In certain embodiments, a vial adaptor for removing liquid contents from a vial comprises a housing and a bag. In some embodiments, an initial volume of a regulating fluid can be contained within the bag. In some embodiments, the regulating fluid flows into the vial as liquid is removed from the vial via the adaptor, thereby regulating pressure within the vial. In some embodiments, a vial adaptor is coupled with the vial in order to remove the liquid. In some embodiments, as the liquid is removed from the vial via the adaptor, the bag expands, and in other embodiments, the bag contracts.

First claim

The following is claimed: 1. A device for transferring fluid to or from a vial, wherein the device comprises: a connector configured to couple the device with a vial, the vial containing a medicinal fluid; a septum piercing assembly comprising a distal regulator aperture and a distal extractor aperture, wherein the septum piercing assembly is configured to pierce a septum of the vial and thereby place the distal regulator aperture and the distal extractor aperture in fluid communication with an interior of the vial when the device is coupled with the vial; an extractor channel connected to the distal extractor aperture and a proximal extractor aperture, wherein the extractor channel is in fluid communication with the interior of the vial when the device is coupled with the vial, the extractor channel configured to allow fluid to be transferred to or from the vial when the device is coupled with the vial; a needle-free medical connector interface connected to the proximal extractor aperture, the needle-free medical connector interface configured to accept a medical device connector; a rigid barrier enclosing a reservoir and separating the reservoir from ambient, wherein the reservoir comprises: a first portion in fluid communication with the interior of the vial when the device is coupled with the vial; and a second portion not in fluid communication with the interior of the vial; a flexible barrier located inside the reservoir, wherein the flexible barrier is configured to divide the reservoir into the first and second portions of the reservoir, the flexible barrier configured to move between a first position and a second position when fluid is added to the vial; and a regulator channel connected to the distal regulator aperture and a proximal regulator aperture, the regulator channel configured to allow a regulating fluid to move between the interior of the vial and the first portion of the reservoir when the device is coupled with the vial; wherein the device is configured such that, when fluid is added to the vial via the extractor channel, regulating fluid moves from the interior of the vial to the first portion of the reservoir via the regulator channel and causes the flexible barrier to expand; and wherein the device is configured such that, when medicinal fluid is removed from the vial via the extractor channel after fluid is added to the vial, regulating fluid moves from the first portion of the reservoir to the interior of the vial via the regulator channel and causes the flexible barrier to contract. 2. The device of claim 1 , further comprising a filter between the interior of the vial and the reservoir when the device is coupled with the vial, the filter configured to prevent liquid from entering the reservoir via the regulator channel. 3. The device of claim 2 , wherein the filter is a hydrophobic filter. 4. The device of claim 1 , wherein the rigid barrier fully encloses the flexible member. 5. The device of claim 1 , wherein the rigid barrier is capable of protecting the flexible barrier. 6. The device of claim 1 , wherein the rigid barrier comprises polycarbonate plastic. 7. The device of claim 1 , wherein the flexible barrier is configured to expand or contract as fluid is inserted into or removed from the vial, thereby equalizing pressure within the vial. 8. The device of claim 1 , wherein the flexible barrier is physically attached to the rigid barrier. 9. The device of claim 1 , wherein the flexible barrier is both circularly symmetric and symmetric about a latitudinal plane passing through a center of the flexible barrier. 10. The device of claim 1 , wherein the extractor channel extends from the septum piercing assembly all the way through the needle-free medical connector interface, such that the needle-free medical connector interface is in fluid communication with the septum piercing assembly via the extractor channel. 11. The device of claim 1 , wherein the septum piercing assembly comprises two piercing members. 12. The device of claim 1 , wherein the septum piercing assembly comprises exactly one piercing member. 13. A method of manufacturing a device for transferring fluid to, or from, a vial, the method comprising: molding a device comprising: a connector configured to couple the device with a vial, the vial containing a medicinal fluid; a septum piercing assembly comprising a distal regulator aperture and a distal extractor aperture, wherein the septum piercing assembly is configured to pierce a septum of the vial and thereby place the distal regulator aperture and the distal extractor aperture in fluid communication with an interior of the vial when the device is coupled with the vial; an extractor channel connected to the distal extractor aperture and a proximal extractor aperture, wherein the extractor channel is in fluid communication with the interior of the vial when the device is coupled with the vial, the extractor channel configured to allow fluid to be transferred to or from the vial when the device is coupled with the vial; a needle-free medical connector interface connected to the proximal extractor aperture, the needle-free medical connector interface configured to accept a medical device connector; a rigid barrier enclosing a reservoir and separating the reservoir from ambient; and a regulator channel connected to the distal regulator aperture and a proximal regulator aperture, the regulator channel configured to allow a regulating fluid to move between the interior of the vial and the reservoir when the device is coupled with the vial; and positioning a flexible barrier within the reservoir, such that: the reservoir is divided into a first portion and a second portion, the first portion in fluid communication with the interior of the vial when the device is coupled with the vial, and the second portion not in fluid communication with the interior of the vial; the flexible barrier is configured to move between a first position and a second position when fluid is added to the vial; and wherein the device is configured such that, when fluid is added to the vial via the extractor channel, regulating fluid moves from the interior of the vial to the first portion of the reservoir via the regulator channel and causes the flexible barrier to expand; and wherein the device is configured such that, when medicinal fluid is removed from the vial via the extractor channel after fluid is added to the vial, regulating fluid moves from the first portion of the reservoir to the interior of the vial via the regulator channel and causes the flexible barrier to contract. 14. The method of claim 13 , further comprising positioning a filter between the interior of the vial and the reservoir when the device is coupled to the vial, the filter configured to prevent liquid from entering the reservoir via the regulator channel. 15. The method of claim 13 , wherein molding comprises molding two or more parts. 16. The method of claim 13 , wherein the rigid barrier comprises two or more molded parts. 17. The method of claim 13 , wherein molding comprises ultrasonic welding two or more molded parts. 18. The method of claim 13 , wherein molding comprises attaching two or more molded parts using a snap fit. 19. The method of claim 13 , wherein molding comprises attaching two or more molded parts using a friction fit. 20.
